Case Manager / Personal Agent

Position:
Support adults with Intellectual /Developmental Disabilities and/or their families, to figure out what they want for their life including supports for employment, leisure, and living assistance.
Develop written goals and plans that support the individual’s life choices.
Assist individuals to acquire the resources they need to live the life they choose.
The position serves people throughout Jackson and Josephine Counties.
Work hours are up to 40 hours per week, 8:00 a.m. – 5:00 p.m. Monday through Thursday and 8:00 a.m. to 2:30 p.m. Fridays with a 30-minute meal break and appropriate rest breaks. Some remote work is an option.

Requirements:
Social Services degree or related field and experience.
Five years’ experience working with people with disabilities could be considered in place of Social Service degree.
Oregon Driver’s License and reliable transportation to coordinate and attend meetings in the community and conduct home visits.
Good computer skills a must
Spanish language fluency is a plus
Must be able to pass a background check and drug screen.
Skills:
Ability to build and maintain positive relations with people with disabilities and their families.
Ability to build quality working relationships with partner agencies and other resources.
Must be able to organize work independently and balance multiple priorities.
Be able to read and apply rules and regulations.

Principle Duties and Responsibilities:
Annual Planning
Utilize the person center planning process in gathering of information to determine needs and development of a support plan for the individual.
Review ONA and other relevant documents
Coordination and Follow up
Provide support consultation for up to 45 individuals, maintains regular contact with individuals and families and monitors progress of the support plan, visits the individual and the family at home at least annually and as appropriate.
Educates the individual and the family on acquiring a variety of goods and services including but not limited to personal care, respite care, job development and coaching, special equipment, transportation or recreational services, etc.
Meets with individuals in person including visits to their home, meetings with other agencies such as VR, schools or other resources in the community.
Educates the individual and family on outside agencies, describes agency and financial options to the customer and/or families.
Educates customers and families about employer of record responsibilities.
Assists in preparing documents and agreements between the customers and their selected employees or providers.
May assist with quality assurance review of service agreements, timesheets, invoices, case notes and other documents on behalf of the customer.
Provides Choice Counseling at least annually.
Resources
Builds relationships with existing community resources, maintains a list of contacts, and facilitates the development of new community resources as requested in the individual support plans.
Documentation
Maintains records of evaluations, progress notes, support plans, referrals, service provisions, and follow up reports for each individual served.
Prepares reports and provides statistical information as required by supervisor.
Professional Conduct
Is respectful of the needs of the work schedule. This includes informing supervisor regularly of anticipated work schedule, using work time productively and using leave time prudently.
Continues to maintain work schedule during pandemic time or other public health emergencies, be it from home, in the formal office or in the community.
Exhibits professional appearance and professional conduct at all times. This includes complying with dress expectations from the personnel policies. It also includes maintaining positive communication that is supportive of the organization and all of the organizations staff and board members.
Is willing to abide by policies and procedures outlined for the agency.
Miscellaneous
Participates in staff meetings, training programs and other meetings as requested by supervisor.
May be asked to work on special projects or assignments as needed or requested.
Completes other duties as assigned.
