What are the responsibilities and job description for the Welcome Family Nurse Home Visitor position at Criterion Child Enrichment?
Welcome Family is a program led by the Massachusetts Department of Public Health to strengthen systems of care for families and children. Welcome Family nurses offer a universal one-time visit and follow-up phone call to all caregivers with newborns, with the goal being to serve as an entry point into the early childhood system of care for families with newborns.
The program is free to parents who have given birth to a child in the city of Springfield, or who are residents of Springfield but gave birth elsewhere. It does not matter if you have other children or if this is your first time, if you meet the residency requirement, you qualify! Just make sure the hospital asks if you would like a Welcome Family home visit prior to discharge.
Job Description:
We are currently seeking a part time (10-15 hours a week) Nurse Home Visitor.
- Conduct up to 90 minute visits at the family's home
- Lactation consulting experience a plus!
- Provide screening, brief intervention, and education to all families, regardless of perceived risk or need.
- Offer referrals to services for caregivers with an identified need, including MHVI-funded home visiting programs and other community resources.
- Address the caregiver's individual questions and concerns during the visit.
- Discuss the contents of the Welcome Family bag with the caregiver.
- Conduct the assessment and present information in an unbiased, culturally appropriate manner, particularly related to sensitive topics.
- Conduct a follow up contact with all caregivers 2-3 weeks post initial visit to document the outcomes of the referrals made during the visit to assess the need for any additional referrals.
- Collect data using the Welcome Family forms.
- Participate in the Welcome Family Learning Collaborative.
- Bilingual applicants are preferred
Qualifications:
- Licensed by the Massachusetts Board of Registration, Division of Professional Licensure with either:
1. Bachelor's Degree in Nursing
OR
2. Master's Degree in Nursing in Maternal and Child Health, Family Health, Community Health, or related speciality
3. Two or more years of professional work experience with prenatal, newborns, pediatrics, lactation, or maternal services
