What are the responsibilities and job description for the CDCA Driver position at CRN Healthcare?

Summary: Provides transportation services that reduce barriers to treatment for clients while supporting and linking operations and housing. Reports housing concerns to the office manager.

Required  to have Active NPI number, Ohio Medicaid Billing Number, Ohio Chemical Board License, Valid Driver's License  

Essential Responsibilities and Duties

Operations:

  • Provides transportation as scheduled for clients, between sober living housing and treatment facility, and to other scheduled appointments

  • Assure agency vehicle is maintained by informing office manager of vehicle needs and completing them as directed by operations manager

Clinical:

  • Develops individualized SUD treatment plans with client input

  • Facilitates/co-facilitates group therapy sessions with clients 

  • Provides individual SUD sessions for substance use disorders

  • Monitors participation and progress of clients assigned to their caseload and engages client in problem solving  when participation or progress is inconsistent with individual treatment plan

  • Assist client as needed with self monitoring crisis situations 

  • Document all clinical services and non-billable client contact in the Electronic Medical Record (EMR)

  • Attends staff meetings as required and participates in weekly clinical supervision 

  • Completes 90 day reviews of treatment plans for each assigned client

  • Completes all documentation in a timely and accurate manner, consistent with legal and ethical requirements

  • Establishes appropriate relationship with clients and client family members that serve to enhance the clients’ capacity for proper self-care and coping while consistently maintaining professional distance and respecting professional boundaries

  • Participates as a member of a multidisciplinary team to optimize client outcomes and treatment effectiveness

  • Maintains required licensure

  • Maintains professional competence through continuous learning, utilizing various methods of self-education and instruction

  • Adheres to all applicable standards and policies of privacy, security, safety, confidentiality and compliance, including applicable laws and regulations (Examples: HIPPA, 42 CFR Part 2)

  • Other duties as assigned
