The Director of Nursing plans, coordinates, leads, controls and evaluates home health and hospice services. The DON directs and supervises professional and paraprofessional personnel rendering client care services, fosters public relations for the company by consulting and planning with health care facilities regarding staffing needs, and communicates the concept of quality client care to the general community.

Responsibilities

  • Serves as the Supervising Nurse of the home health department and assesses appropriate staff supervision during all service hours.
  • Represents the Agency/Department in audits of regulatory agencies.
  • Oversees the implementation and evaluation of client care services inclusive of regulatory licensure and certification criteria and accreditation standards.
  • Supervises and manages the clinical operations of the home health as must be available at all times during operating hours as well as being available by phone for clinical support after hours and weekends, or have qualified alternate in writing.
  • Consults, plans and works with DOS/administrator in process formulation and implementation including assessing adequate and qualified staffing.
  • Coordinates with other program areas and senior management as appropriate according to the organizational structure.
  • Directs the implementation of service goals and objectives.
  • Directs staff in performance of their duties.
  • Keeps home health personnel up to date with clinical information and practices through staff development to include new hire orientation,yearly in-service educations, and continuing education.
  • Assures the field personnel adhere to federal, state, and/or accreditation regulation, and company policy and procedures.
  • Actively develops and pursues referral sources, as well as facilitates on-going relationships with various medical and health communities.
  • Responsible for implementing standards that ensure safe and effective services to patients and families to include:
    • timely admission of all referrals as per the conditions of participation and regulations of home health services
  • Ensure reassessment of a patient’s needs is performed and meets criteria for continued home health services by an appropriate health care professional when:
    • there is a significant health status change in the clients' condition
    • at the Physician's request
    • after hospitalization
  • Responsible for assuring all physician orders are implemented appropriately and timely.
  • Consults with physicians in matters relating to patient care services.
  • Responsible for reviewing and notification of appropriate lab results to physician and care team.
  • Responsible for meeting Hemoglobin A1C requirements.
  • Responsible for final review of OASIS Assessment and plan of care for accuracy and reimbursement purposes.
  • Ensures effective and efficient client care documentation system to support medical necessity and homebound status of patients.
  • Assists in the evaluation of community needs and plans programs and services to meet with identified needs.
  • Consults with supervisors and staff, individually or in groups, regarding patients and/or families.
  • Responsible for the timely completion and providing the field personnel a weekly schedule utilizing the EMR software to assure compliance with the physician ordered frequency.
  • Responsible for ordering and maintaining medical supplies within budget.
  • Responsible for extracting reports, analyzing those reports, and developing weekly actions for improving agency operations.
  • Supervises and evaluates the program, services, and field personnel.
  • Evaluates the performance of all clinical personnel to include:
    • Timely Performance evaluations with measurable goals: Initial, 90-days and yearly.
    • Quarterly clinical staff evaluations.
    • Competency evaluations: initial and yearly.
    • On-site yearly evaluations.
  • Acts as liaison with Staff Development for program areas.
  • Oversees the QAPI program to assure that all components are completed accurately and timely in accordance with QAPI Policy and Procedures manual.
  • Coordinates with continuous Quality Improvement Committee and implements corrective action plans and controls.
  • Assists in agency evaluation activities.
  • Participates with other health, civic, education and professional groups as directed by the DOS/Administrator.
  • Serves on Professional Advisory Committee and periodically reviews policies relevant to patient care. Conducts case conferences as required.
  • Review and implement the agency emergency preparedness needs.
  • Participates in local, state, and national meeting and conventions when directed.
  • Responsible for orientation and training of field personnel.
  • Keeps current on local area issues and assists in organizing, participating in agency events, and/or community awareness of services, as directed by the DOS/Administrator.
  • Participates on the on-call rotation/duties as defined by the on-call policy and as directed by the DOS.
  • Responsible for preparing and reviewing any claims requested by payers.
  • Performs other duties as assigned by the DOS/Administrator.

Driving & Other Requirements

Must have and maintain a valid state driver license, have a registered and reliable vehicle that meets state law standards, and meets the insurance standards of Company. Position requires travel between company sites as well as the community. Traveling by car or airplanes to local or out-of-town meetings, seminars, and conferences.

Education, Experience & Job Requirements

This position requires at least the following minimum requirements:

  • Current and valid Registered Nursing license in the state(s) practicing.
  • Bachelor degree in nursing preferred but not required.
  • Experience as a home health and/or hospice professional with training and experience in health service administration.
  • Preferred but not required at least one (1) year of supervisory or administrative experience in home health care, hospice or related health programs.
  • Preferred but not required one (1) year of experience in personnel management, including hiring, supervision, evaluation and benefits administration.
  • Current CPR Certification.
  • Proven skills in business and financial management preferred.
  • Good nursing judgment.
  • Must be proficient in clinical nursing skills and be able to train/teach field staff.
  • Responsible for ensuring that all agency employees review and comply with federal, state, and/or accreditation regulations, and agency policies.
  • Demonstrated ability to work with patients and employees.
  • Demonstrated ability to work in a proactively diverse and inclusive organization.
  • Must have excellent written communication by writing clearly and informatively, edits work and able to read and interpret legal written information.
  • Demonstrates accuracy, thoroughness and follows through on commitments.
  • Must practice a high level of confidentiality and be able to effectively response to questions from employees, managers and outside entities.
  • Must have excellent communication skills, both verbal and written.
  • Must be able to work with others to resolve issues and maintain composure in an environment of changing priorities.
  • Must have knowledge of all company policies and procedures.
  • Must be able to work on a timetable, follow instructions, responds to management direction and solicit feedback to improve performance.
  • Should have excellent character references and solid work background.
  • Must be able to read, write and speak English. Spanish is preferred.
  • Must have knowledge and proficiency of office computer equipment and software.
  • Demonstrate ability to multi task and work in a fast-paced office setting.
  • Proven ability to cope with conflict, stress and crisis situations.
