Overview
The Advanced Practice Provider works under the general supervision of the Practice Supervisor(s) and Medical supervision of the physician(s) to provide excellent medical care completing the duties and responsibilities of the position while complying with HIPAA and hospital confidentiality policies. They are responsible to provide health care services as licensed professionals working within their scope of practice and in accordance with established protocol and procedures. This position requires regular use of independent judgment. The Patient population ranges from infants to geriatrics.
Responsibilities
Completes a relevant history and physical examination and documents findings on Medical Record.
Orders/Interprets appropriate diagnostic tests and documents in the medical record.
Formulates clinical impression/diagnosis based on clinical findings
Initiates and documents appropriate treatment plan.
Provide quality, compassionate and empathetic medical care to all patients as delegated by a supervising physician within the education, training and experience of the Advanced Practice provider as delineated by protocols, practice guideline and/or practice directives while meeting the productivity demands of the practice. Services may be provided in the office setting, nursing home(s) or CVPH. This includes but is not limited to: Obtain patient medical histories. Perform medical exams. Order or perform diagnostic and therapeutic procedures. Evaluate, diagnose and formulates appropriate treatment plan. Monitor the effectiveness of therapeutic interventions and treatment plan. Provide counseling and health education to patients and families as needed. Prescribe medications as needed and dispense medication samples when available. Make appropriate referrals. Provide emergency care as needed. Document patient records in a manner to support level of service being billed.
Develops of a discharge plan for patients under their care, and communicates/educates patient and family regarding plan.
Works collaboratively with physicians, nursing and other support staff. Routinely consults with physicians.
Participates in department/hospital-wide committees as requested.
Demonstrates commitment to continued growth through attendance at educational programs.
Maintains working knowledge of the various stages of growth development and health status of this wide population. Utilizes appropriate resources.
In conjunction with the Director of Practice Operations and/or Practice Supervisor(s), assist with evaluation of effectiveness and efficiency of practice operations as it relates to patient flow, provision of medical care, risk management and quality assurance.
Regularly attends Department meetings
Other duties as assigned.
Qualifications
Current license or license eligible as a Nurse Practitioner or Physician Assistant in New York State required at time of application.
Applicant must obtain Nurse Practitioner or Physician Assistant certification by start date.
Current New York State DEA certification required by start date.
Previous experience as Nurse Practitioner or Physician Assistant with a focus on adult and pediatric primary care preferred.
As applicable, the individual has training/competency in attending to the special needs and/or behaviors appropriate to the age of the patients for which care is being provided.
