Job Description
The Goal of the program is to remove barriers that our members face due to Social Determinants of Health issues, access to care and how to access needed services for their health care needs. This will be done in a linguistically, culturally appropriate manner
*Must live in Kings County and be able to travel throughout Kings with trips into Queens & Bronx for community events*
Community Health Care Workers (CHW) serve the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. By providing these services, CHWs help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other. This position is part of a new initiative in addressing members needs who are most vulnerable.
As a Community Health Care Worker at Aetna you will:
• Work on an extended larger team that includes a Community Health Educator (CHE), Nurse Care Managers, and other CHW’s. As a team you will work to provide needed resources, fill gaps, and remove barriers so that the member will be successful in managing their health care needs.
• Meet with members in their home to conduct a brief Needs Assessment
• Participate at local vaccination community events (Covid, Flu, Shingles)
• Participate in health fairs that provide BP,A1C, cholesterol
• screenings.
• Collaboration with CBOs and Providers (FQHC, Dept of Health, traditional providers)
• Reinforcement of health care messaging in linguistically appropriate language
• Service coordination and navigation
• Assist members with program application completion that addresses Social Determinant of Health issues and barriers
• Educate members on Life Skills such as budgeting, food shopping, food preparation
• Educate members on Healthy Living, Physical and mental health well begin, Tobacco free drugs, alcohol.
• Assist the member with referrals for services in the community
• Document work with member in a database system and review as a team
Pay Range
The typical pay range for this role is:
Minimum: 18.50
Maximum: 34.60
Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.
Required Qualifications
Must live in Kings County and be able to travel throughout Kings with trips into Queens & Bronx for community events
Preferred Qualifications
Bilingual/target population preferred
• 2-3 years’ experience providing clinical services; experience in community/outpatient setting preferred.
• Community Health Worker Certification or equivalent training and experience preferred but not required
• Flexibility in work settings like the members home or other community setting
• Experience in serving the Medicare Populations in urban or rural environments with familiarity of resources within Kings, Queens & Bronx Counties, NY
• Preferred skills: Health Care Condition knowledge like Diabetes, Hypertension, Cardiovascular Disease
• Experience using a computer including the use of Excel and Electronic Health Records
Education
• High School Diploma or G.E.D.
