What are the core duties for this position?
The essential job functions for this position involve providing counseling and support to program participants or workers. The role includes various responsibilities such as:
- Direct Counseling: Providing counseling to program participants or workers.
- Individual Written Rehab Plan: Developing performance goals and behavioral criteria in accordance with CARF standards through an Individual Written Rehab Plan (IWRP) with input from Employment Specialists/Production Supervisors.
- Performance Evaluation: Regularly evaluating the work performance and social adaptation of participants, maintaining records, and providing feedback on their progress.
- Readiness for Placement: Monitoring and reporting on participants' readiness for job placement and collaborating with a Job Developer to enhance job-seeking skills.
- Documentation: Writing discharge summaries, conducting quarterly reviews, and preparing necessary documentation, including reports for relevant social service agencies.
- Recruitment and Intake: Soliciting and recruiting prospective participants, conducting intake interviews.
- Plan Modification: Revising and modifying rehabilitation plans based on progress evaluations.
- Reporting: Preparing and submitting required reports to relevant government agencies.
- Representation: Representing the organization at conferences, seminars, and meetings related to rehabilitation activities.
- Data Monitoring: Keeping track of statistical data related to department goals and planning.
- Personal Care: May involve attending to personal care issues of participants.

While performing the duties of this job the incumbent will have to sit, stand and or walk for the majority of the shift. The incumbent is required to regularly talk or hear while fulfilling the duties of the job. The incumbent may have to lift up to 25 pounds and occasionally lift and move more than 75 pounds. The incumbent must have a clean driving record as they may have to act as a backup driver.
