QUALIFICATIONS
1. High school diploma or G.E.D. is required.
2. Must be at least 18 years of age.
3. One year of experience working with customers/clients is required. Formal education in behavioral health may be substituted.
4. Two years of experience in a health care setting is preferred.
5. American Heart Association or American Red Cross Certification in Basic Life Support for Healthcare Providers is required.
6. Ability to speak, read, and write English is required.
CORE RESPONSIBILITIES

SPECIFIC RESPONSIBILITIES AND DUTIES
1. Complies with:
a. Prescribers’ Orders;
a. Hospital Policies & Procedures;
b. State & Federal Regulations;
c. State & Federal Statutes; and
2. Demonstrates teamwork by carrying out routine and assigned tasks within the program.
3. Assists in provision of maintaining a safe and therapeutic milieu by monitoring and complying with program rules and hospital policies.
4. Observes patients in accordance with prescriber orders and hospital policy; documents and reports findings to supervisor immediately if behavior and/or activity is significant.
5. Conducts and documents safety rounds per policy.
6. Provides assistance in management of patients who exhibit escalating behavior, in accordance with hospital training and policy.
7. Documents patient activities, significant occurrences, observed behavior, vital signs, and other findings as assigned by the nursing staff and determined by hospital policy.
8. Assists patients in completing Activities of Daily Living (ADLs), as needed. This may include hygiene, grooming, toileting, eating, and other functions.
9. Displays flexibility and willingness to work with others.
10. Identifies opportunities for improvement and offers solutions; discusses with appropriate personnel (i.e., supervisors).
11. Consistently demonstrates vision, foresight, good judgement, and customer sensitivity.
12. Models productive, healthy behavior and interactions with co-workers, patients, and others.
13. Performs job with efficiency necessary to complete assignments within established time constraints and procedural guidelines.
14. Organizes and prioritizes work while being cost-effective and conserving resources.

16. Offers and accepts constructive feedback and responds appropriately to direction.
17. Is punctual in all aspects of work, and adheres to attendance policy.

21. Actively participates in:
a. Identifying opportunities for improvement to be included in the Quality Assurance Performance Improvement (QAPI) plan;
b. Collecting QAPI data, as assigned;
c. Hospital Committees; and
d. Staff Meetings
22. Immediately reports alleged, suspected, or witnessed abuse/neglect and all unusual behaviors and occurrences to LPN or Charge Nurse.
23. Performs other duties and special projects as required.
