Care coordination is the deliberate organization of patient care activities between two or more participants (including the patient) involved in a patient#s care to facilitate the appropriate delivery of healthcare and social services needs. This position requires excellent time management skills, the ability#to link to the resources needed to carry out all required patient care activities. Also required is the management and exchange of information among participants responsible for different aspects of health care and other social determinants of health. Collaborative procceses include assessing, planning, facilitation, care coordination, evaluation and advocacy to meet the comprehensive needs of individuals and families. The position requires #a demonstrated ability to work with patients across the life span providing,#culturally sensitive care.# This professional works as an integral member of a team-based care system within the Patient Centered Medical Home, Blueprint for Health, OneCare Vermont and other community partners.
