What are the responsibilities and job description for the Elder Law Care Coordinator position at DeLoach Hofstra & Cavonis, P.A.?
Our elder law department is looking for a Care Coordinator. As part of our elder law practice, we help our families with healthcare advocacy, guiding our clients and their families through the legal, financial and healthcare aspects of the aging process.
Skills required are as follows:
- Social Worker or other medical background in long-term care necessary
- Local knowledge of healthcare providers and long-term care facilities
- Familiarity with health insurance and Medicare
- Computer skills
- Self-directed, time management and attentive
- Good people skills, especially in times of crisis
- Willing to learn and grow as a health care advocate
Duties are generally as follows:
I. Care Coordination
- Responsible for conveying appropriate medical advocacy to address client concerns in a compassionate and caring manner considering at all times the stress level of the family.
- Responsible for obtaining all necessary physician or health care facility forms needed for public benefits.
- Responsible for in person medical/care assessment and recommendations therefrom.
- Responsible for helping locate suitable long-term care facilities for clients to tour, checking on bed availability, price and level of care.
- Responsible for communicating with long-term care facilities on client placement, care issues and move-in procedure, particularly regarding health and billing issues.
- Responsible for educating clients and family on questions and procedures to assist with client’s care.
- Responsible for advocating, communication and assisting with communication with healthcare providers.
II. Education and Marketing – Time Permitting
- Responsible for educating medical professionals about the services law firm provides.
- Responsible for making contact with administrators and staff when visiting clients in facilities for marketing purposes.
- Responsible for attending marketing groups getting firm message out.
- Responsible for tracking marketing contacts and referrals and reporting to Director of Marketing on efforts.
Salary based upon skill and experience.
