General Description

Under direction of the LIDDA Director, and the PASRR Supervisor, and within federal, state and local guidelines, performs routine work meeting the needs of individuals with intellectual and developmental disabilities residing in Nursing Facilities.

The PASRR Habilitation Coordinator reviews information and performs assessments of those individuals who are referred to and/or residing in a nursing facility to ensure a least restrictive living environment. This process is to ensure the appropriate admittance into nursing facilities and provides assistance in obtaining alternative living opportunities for those who request alternative housing. This person coordinates all specialized services; such as, day programs, independent living skills, physical therapy, etc. for the individual. The person in this position obtains and reviews medical records and history of those referred to or residing in nursing facilities who have a history of an Intellectual Developmental Disability and/or Mental illness.

Job responsibilities are conducted in accordance with the Texas Administrative Code (TAC), HHSC Performance Contract, HHSC program specific Handbooks, Medicaid requirements, Center policies and procedures as well as other applicable state and federal laws. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment. Organizational skills are a must, along with critical thinking abilities.

Education, Training and Experience (including licensure and certification)

This position requires a bachelor's, or advanced degree from an accredited college or university with a major in social, behavioral, or human service field, such as psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice; and, must have at least one year of experience working directly with individuals with intellectual or developmental disabilities in order to meet the requirements of a QIDP. MHMR or Service Coordination experience is preferred.

Knowledge, Skills and Abilities

  • Knowledge of rehabilitation programs for individuals with developmental disabilities. Extensive experience in providing service coordination to individuals with IDD, including those who have complex medical needs.
  • Skill in the operation of computers and in applicable computer software.
  • Ability to collect and analyze data, to design therapeutic programs, to prepare and maintain records, and to communicate effectively.

Essential Duties

  • Monitors the TMHP portal and responds to all IDD PASRR Evaluation alerts within designated timeframes.
  • Facilitates Service Planning Team meetings at least quarterly, or more frequently if requested, or if there is a change in service needs, within the designated time frames.
  • Attends and participates in nursing facility initial IDT and Care Plan meetings.
  • Enters and approves data in the TMHP portal within the designated timeframes.
  • Develops and revises the Habilitation Service Plan for each individual in a timely manner.
  • Monitors the Individualized Service Plan and implementation of personal outcomes, services and supports including services through various funding sources.
  • Completes a review of Community Living Options with all assigned individuals at least semiannually.
  • Assists in transition planning for individuals moving from nursing facilities into the community and assists with nursing facility diversion requests.
  • Documents monthly face to face contact with all assigned individuals on the caseload.
  • Maintains positive relationships with individuals, family members, service providers, Center employees, other agencies and the community.
  • Completes all required documentation as required and in accordance with Center policies and procedures, HHSC contract, Medicaid requirements, the TAC and applicable HHSC handbooks.
  • Responds to crisis intervention with minimal supervision.
  • Provides back up to other Habilitation Coordinators during absences or vacancies.
  • Works both independently and as part of a team.
  • Perform additional duties as deemed necessary by department supervisor within designated timeframes.
