Deschutes County Health Services supports over 400 staff in Behavioral and Public Health programs working to promote and protect the health and safety of the community in a broad variety of community-based settings.
 
This position is part of Public Health service area, which works to keep people healthy and safe by striving to improve and protect the health of people in Deschutes County. This work includes: disease prevention, surveillance and control; food and consumer safety; assuring clean and safe drinking water; ensuring access to essential health services; providing education about healthy lifestyles; implementing strategies to reduce and prevent high-risk behaviors among youth; and responding to pandemics and emerging threats, outbreaks and other events.

ABOUT THE JOB:
Deschutes County is accepting applications for a Public Health Nurse III on the Nurse-Family Support Services team within Clinical & Family Services. Nurse-Family Support Services provides personal nurse visits that include comprehensive maternal and infant assessments, case management, linkage to community resources, and parent education and skills building. Programs include CaCoon (case management for children with special health care needs), Babies First (prenatal, post-partum and children 0-5 home visiting), and Family Connects (universally-offered new baby home visiting). This position will be focused on Family Connects services in Deschutes County. Family Connects is an evidence-based program, supported by the Oregon Health Authority.  This position will provide home visiting services for newborns and their families, team leadership, and program-related work to grow Family Connects within our community. The PHN III will work closely with the Nurse-Family Support Supervisor to ensure Family Connects Deschutes is meeting goals and metrics, utilizing data to make program improvements, and working with community partners to successfully integrate Family Connects into routine newborn and post-partum care in Deschutes County.  This position requires travel throughout Deschutes County including Bend, Redmond, Sisters and La Pine. Cross-training in other nurse home visiting programs may be required as appropriate based on department needs.
 
 
 Key Responsibilities: 
  • Complete maternal and newborn nursing assessments to evaluate the health status of newborns, postpartum mothers and/or caregivers. Documenting visit encounters to include nursing assessment, nursing diagnosis, planning, implementation, and evaluation of care in our EHR and data collecting systems.
  • Provide postpartum and newborn education.
  • Provide care coordination and referrals for families to community resources.
  • Collaborate with Clinical & Family Services’ leadership, Oregon Health Authority, and regional and community partners to grow and strengthen the Family Connects program in Deschutes County.  This may include: developing & implementing new strategies, monitoring and utilizing program data, and identifying and addressing quality improvement needs.
  • Monitor, document, and report on program success, quality assurance activities, auditing work to ensure compliance with program and state regulations, and analyzing data from program and report on effectiveness and outcomes.

Knowledge of or experience with:
  • Working with high-risk clients in the maternal child health nursing field.
  • Establishing therapeutic relationships with clients, demonstrating excellent communication and listening skills.
  • Charting in electronic health record systems.
  • Leading, developing and/or implementing strategies to grow and strengthen new programs.
Skill in:
  • Communicate effectively both verbally and in writing.
  • Establish and maintain effective working relationships with community agencies and partners, the general public, and other employees at all times.
  • Maintain confidentiality.
  • Work independently as well as in a team setting.
  • Be self-motivated and resourceful, and have the ability to keep organized with good time management skills.
  • Show understanding of cultural competency and problem solving skills.

  • Associate degree in Nursing; 
  • AND five (5) years of relevant experience in public health or related nursing experience; 
  • OR any equivalent combination of training, education, and experience that provides the required skills and knowledge to perform the essential functions of the job. 

Preferred Qualifications:

  • A Bachelor's degree in Nursing with five (5) years of public health or maternal child health nursing experience.
  • Bilingual in English and Spanish.

Driving is a requirement for this position. Possession of or ability to obtain a valid Oregon driver's license within 30 days of hire date. The employment offer will be contingent upon presentation of an acceptable and verifiable driver's license, pre-employment screening for criminal history, driving history, and controlled substances (NOTE: Positive test results for marijuana use may result in rescission of a contingent offer of employment). This screening must be completed with satisfactory findings in order for a formal offer of employment to be extended.

Must have dependable transportation and be able and willing to drive throughout Deschutes County in sometimes adverse weather conditions.

Must be willing to work a flexible schedule when necessary to meet the needs of the department. Limited remote work may be available upon approval.
 
Possession of or ability to obtain an Oregon State Registered Nurse License (in good standing) by time of hire.
 
Sufficient ambulatory ability to deliver services throughout Deschutes County, including the materials needed for presentations, and to provide face-to-face services in a variety of settings in the community, including but not limited to hospitals, schools, jails, clinics, client homes, and community and residential settings within required response times.
 
Physical Requirements. Lift and carry equipment up to, but not exceeding 30 pounds. Climb stairs, stand for extended periods, walk distances of 100 feet, and sit down on the floor and get back up without assistance.
