The Nurse Case Manager Specialist / RN assists in Case Management and supporting patient health initiatives throughout Discount Tire and America’s Tire regions
Essential Duties and Responsibilities:
- Provide comprehensive case management services in a timely manner, including, but not limited to the following: intake assessment, communicating with other staff and health professionals, obtaining necessary authorizations, transfer of medical records, and providing additional medical resources.
- Work with Discount Tire staff to help navigate and comprehend medical terminology, physician diagnoses and recommendations.
- Participate in the Executive Health Department initiatives and programs.
- Contribute and lead departmental meetings and trainings.
- Liaison between Discount Tire and physicians/hospitals.
- Maintain confidentiality; maintain current on required training and certifications.
- Maintain, update and keep accurate documentation.
- Adhere to company policy, procedures and professional code of ethics
- Participate in departmental staff meetings, training and accept responsibility for aiding in the development of positive team relationships.
- Learn, live and promote the Discount Tire culture.
Qualifications:
- Registered Nurse licensed in the State of Arizona.
- Ten years of recent consecutive nursing practice.
- Bachelor of Science in Nursing degree strongly preferred
- Excellent written and verbal communication skills.
- Experience working with individuals in crisis.
- Organized and able to handle a large case load with ongoing follow up.
- Excellent organizational skills and detail oriented.
