What are the responsibilities and job description for the Social Worker position at District Health Department #10?
Full-time Social Worker. This position will be based in Lake or Mecosta County.
Duties:
- With a team of RN’s, SW’s and RD’s, the Maternal Infant Health Program (MIHP) promotes healthy pregnancies, positive birth outcomes, and infant growth and development.
- During home, office, and tele-health visits, provide assistance to help families support their basic needs, offer prenatal and parenting education, and referrals to community services.
- The CSHCS program works with families to enable individuals with special health care needs to gain improved health outcomes and an enhanced quality of life.
Requirements:
- Knowledge of social work theories, principles and practices acquired through completion of a Bachelor’s degree from accredited school of school work.
- Master’s degree preferred.
Advanced social work skills and knowledge of public health policies, procedures, and practices at a level normally acquired through two years of related, professional experience.
- Current Michigan social work license.
- Ability to enter, retrieve and manage client data, notes and care plans in electronic health records, and other on-line or network databases or logs.
Job Type: Full-time
Pay: $24.00 - $28.00 per hour
