Doors of Hope Geauga is seeking a part-time or full-time Case Manager to support program leadership in providing case management and other support services to families in shelter.
Principal Responsibilities
Under the direction of program leadership, provides direct service case management, including assessment, housing/stability planning, linkage, and follow-up to families in shelter on a daily basis.
Monitors case plans and ensures families participate in services and programs to achieve goals.
With program leadership, assists families in successful transition from shelter to independent living in the community, including move-out planning, securing of necessary resources, and development of landlord/tenant relationships.
Maintains client case records in a manner consistent with best practices and confidentiality guidelines.
Documents client progress and reviews cases with supervisors as required.
Under the direction of program leadership, ensure that agency rules and policies are followed, and client responsibilities are fulfilled.
Assists in maintaining a safe and welcoming shelter environment.
Participate in training as required by program leadership.
Ensures the family living units are well maintained and necessary supplies are provided.
Other duties as assigned by supervisor and Board of Directors.
Qualifications
Minimum of two years’ experience in case management working with children and families preferred.
Minimum of high school education with additional training or associate’s degree preferred.

Ability to build rapport quickly with clients, volunteers, and other staff through strong interpersonal skills.
Ability to take direction and work effectively, professionally, and respectfully with colleagues and clients.
Basic skills in Microsoft Office Suite.
Ability to work flexible hours; some evening and weekend hours are required.
Valid driver’s license and insured vehicle. Some local travel will be required.
