Join our team as an admissions coordinator!
Working as an admissions coordinator, you will be responsible for:
- assessing patient medical and financial information
- working cooperatively with the multidisciplinary team to ensure a smooth admission transition
- coordinating patient admission with hospital d/c planners and other referral sources
- ensuring continuity of care
- developing and maintaining excellent relationships with referral sources
Required Qualifications
- degree in social work or related healthcare field, such as nursing (RN or LPN), physical therapist assistant (PTA), or occupational therapy assistant (COTA)
- ability to multitask, manage multiple priorities, and work in a fast-paced environment
- knowledge of various computer applications (experience with EPIC is helpful, but not required)
- detailed oriented with strong organizational and communication skills
