What are the responsibilities and job description for the Case Manager position at DREAAM?
JOB SUMMARY:
The Case Manager I is responsible for appropriate screening and enrolling youth into DREAAM programs. Leading life skills and youth development workshops, and developing Individual Plans to assist the youth, family, and team in establishing a pathway to create success and obtain goals. The Case Manager I will work primarily with the DREAAM Makers Program Director to accomplish the established program goals. There is a 90 day probationary period for this position.
REQUIRED QUALIFICATIONS:
- Bachelor’s degree in social work, education, psychology, or related field
- Must pass background check
- Valid driver’s license and auto insurance if the candidate has an automobile
- Minimum of one to two years of professional, full-time experience in youth development and direct service activities or equivalent experience
- Demonstrated ability to provide outcome-driven program planning and implementation
- Excellent organizational and communication skills (oral and written)
- Strong leadership abilities. An ideal candidate will be able to inspire and bring out the best in young people, the team, and the volunteers
- Evidence of cultural competence when working in African American communities
- Ability to deal effectively with youth across ages and demonstrate a working knowledge
of trauma-informed care.
ESSENTIAL JOB RESPONSIBILITIES:
- Facilitates job training workshops and teaches leadership and employability courses
- Supervises and leads youth development services for youth at the Day Reporting Center
- Completes daily activity log and submits to Program Director
- Carries a caseload that may be up to 50 clients, varying on program census
- Enrolls participants in IYIP and WIOA
- Maintains case file documentation in accordance with specific grant policies and procedures
- Maintains confidentiality when documenting, consulting, or consolidating information
necessary for case management.
- Completes at a minimum monthly case notes for each participant on the caseload
- Plans coordination of care including, but not limited to, school records, medical records,
mental health treatment, legal status, court involvement, social and family strengths
and needs, support systems, and specific cultural issues
- Maintains regular contact with the family, service providers, and internal DREAAM staff
to provide consistent coordination of services and progress
- Ensures assessment compliance of participants
- Participates in team meetings, staff in-service training, and department meetings
- Completes Signification Incident Reports (SIRs) per federal policy requirements
- Conducts internal chart reviews to monitor documentation and equipment inventory compliance
- Completes timely and accurate timesheets, required training, and other personnel/administrative records as required
- Maintains program supply list
