Education & Experience Requirements:
Bachelor’s degree in psychology or related field and at least 1 year of relevant work experience; or at least 5 years of relevant work experience.
Primary Responsibilities:
Connect youth and family to needed resources, services, and activities while supporting youth and team-defined Personal Plan goals.
Maintain a clear and complete Success Coach Position Folder.
Review applications and conduct interviews to recommend to the Program Supervisor if a youth will be accepted into our TLP .
Maintain clear, complete, and timely file documentation for each youth.
Meet with incoming residents, and parent/guardians, to complete intake paperwork.
Meet with each participant to set and review goals in individually-designed Personal/Transition Plans, focusing on education, employment, good tenancy, health, accessing mainstream benefits, and life skills.
Meet with each participant at a minimum once a week for 1 hour to monitor youth progress toward goals and to support youth with barriers and setbacks and coach them toward successful accomplishment of each goal.
Provide transportation under exceptional circumstances only (i.e., initial orientation, appointments that mandate DTP presence, scheduled group activities, and urgent medical situations).
Implement program rules and policies, including creating appropriate behavioral contracts when needed.
Perform urinalysis tests as required.
Perform casita searches for prohibited items as scheduled and as needed.
Conduct exit interviews with youth.
Maintain satisfactory working relationships with representatives of employers, area schools, colleges, GED programs, financial aid counselors, parent/guardians, behavioral health partners, and other persons essential to success.
Attend one-on-one meetings with Program Supervisor as scheduled.
Attend weekly Staff Meeting, Clinical Supervision, and TLP and Shelter team meetings.
Coordinate independent living skills classes, community events, purchasing supplies
After Care tracking
Maintain volunteers for TLP
Facilitate Shelter Youth Council
Maintain a clean driving record based on DreamTree Project insurance requirements.
