The Internal Medicine Department at Duly Health and Care is seeking a Full Time, Board Certified/Board Eligible Internal Medicine Physician to join our busy and growing practice located in Willowbrook.
Opportunity to practice value based medicine with a growing Medicare Advantage patient population- See patients in the clinic and post hospital discharge for rehab in long term care facilities
- Average week includes 3-4 1/2 days caring for nursing home rehab patients and the remainder of the time in our Willowbrook office
- Fully ramped practice is 8-12 patients in nursing home per 1/2 day and 8-12 patients per 1/2 day in clinic
- Call is mostly by phone due to hospitalist coverage
- Large physician group, RN and MA support
- Flexible schedule
- Shareholder track
- Epic EMR
