As a member of the interdisciplinary care team, this position is responsible for providing administrative and provider support in the delivery of quality behavioral health care for patients with DVCH's Medication-Assisted Treatment Addiction Clinic (MAT). Provides care to patients with special awareness of vulnerable populations treated in the health centers that may face barriers to care due to residence, health, age, personal characteristics, ability to communicate, chronic illnesses and comorbidities, high ER utilization, developmental status or financial status.
Job Responsibilities:
Assists with developing and maintaining patient electronic records.
Serves as a liaison between the clinic and community agencies/facilities for the exchange of patient related clinical/referral information.
Assists with educating and coaching patients about behavioral changes on specific topics as instructed by practice guidelines.
Responsible for obtaining discharge documentation for DVCH MAT patients with transitions in care from area hospitals/other providers and coordinates continuity of appropriate follow-up patient care.
Sorts, scans, and electronically files paper documents in the patient's electronic health record.
Schedules and reschedules patient appointments.
Returns the daily receipts worksheet and copayments collected to the Health Center Manager or designee at the end of each day.
Participates in community-based health fairs and other projects.
Translates for providers and patients, when applicable.
Qualifications, Education, and Knowledge:
A high school diploma or equivalent is required.
Graduation from an accredited Medical Assistant program is preferred. A combination of education and experience will be considered.
Bilingual (English/Spanish) skills is required.
Ability to work effectively with all levels of care team and other colleagues.
Demonstrated ability to interpret and apply guidelines and procedures.
Proficiency in basic computer skills.
Ability to work with electronic health records (EHR).
Must possess strong interpersonal skills.
