Summary: Community Integration QIDP works with individuals who have intellectual or developmental disabilities. Community Integration QIDP’s assist individuals to become more integrated in their communities by maximizing the person’s strengths, interests and abilities. The QIDP is responsible for ensuring that individuals have the opportunities to be integrated in their communities through activities and self-advocacy. .Support may occur in a variety of settings and may include providing personal care, assistance with daily living activities, job coaching, teaching/developing skills, advocating with the individual, assisting in communication and self- expression, and ultimately supporting the individual in meeting his or her own goals
Duties and Responsibilities:
1. Maintain compliance with all regulatory bodies
- Supervises assigned staff in compliance with Easter Seals Arc Human Resources policies, BDDS/Medicaid regulations, ICF/MR regulations and EE0/AA guidelines.
- Trains staff in compliance with ESARC HR Human Resources Policies, EEO/AA guidelines and Medicaid Regulations.
- Adheres to ESARC Human Resources and EEO/AA policies, attends meetings/in-services and performs other duties as assigned.
- Maintains detailed client case notes, compiles and submits reports.
- Accurately tracks, monitors and documents all approved billing
- Ensures accurate and proper documentation of services; ensures that services adhere to regulations
- Maintains documentation for assigned programs and regularly reports to Director of Call Center and Recreation Activities.
2. Ensure services provided are individualized
- Plans and implements community activities according to individual clients interest and support plans.
- Researches community events and communicates to staff in order to implement
- Monitors for progress and ensures follow through with objectives listed in the individual’s ISP and BSP
- Provides crisis intervention/supportive counseling, assists with supervision of clients as needed.
3. Develop a workforce that fosters working through a continuum of services
- Supervises assigned personnel, including but not limited to; orienting, training, disciplining and terminating, reviewing and approving work schedules. Shall perform duties of absent personnel as needed.
- Ensures that all staff is properly trained prior to working with participants and that the training is documented according to department policy.
- Ensure community integration coordinators are partnering with residential staff and clients to carry out individualized person centered plans.
4. Support people to be independent and make informed choices
- Ensures individuals health, safety and welfare needs are met while out in the community by informing them of health options, following risk plans and knowledge of environmental hazards.
- Identify, support, and train all staff on individualized person centered goals and ways in which to assist the individual lead a more independent lifestyle
5. Positively represent Easterseals Arc.
- Uphold agency values and follow agency policies and procedures
- Procure volunteers and family members and ensures staff supervision for community activities
- Maintain confidentiality of records/information according to HIPAA, State, Federal Laws and guidelines
- Maximize and responsibly utilize agency and community resources
- Serves as liaison to participants, families and community resources
- Serves as liaison to State and local agencies
Qualifications:
- At least 18 years of age
- High School diploma or GED
- Experience working with individuals with autism and other developmental disabilities; prefer 3-5 years.
- Ability to pass criminal/registry background checks and other screening guidelines as deemed necessary.
- A valid driver’s license, insured vehicle, and safe driving record.
- Basic language, written and computer skills necessary for communication and documentation
- Ability and willingness to support individuals with challenging behavior and/or medical concerns
- Ability to pass and maintain required trainings such as CPR/First Aid, Medication Administration and Positive Behavioral Support Training and other trainings as deemed necessary by the agency
Working Conditions: QIDP’s work in a changing environment that adjusts to the needs of the individuals in program services. The environment may be fast-paced with interruptions, distractions and deadlines. QIDP’s need to be flexible and be able to adjust to changing needs activities and locations. Some site locations may have tobacco smoke and/or pet dander based on the individual being supported. QIDP’s must have an insured, safe vehicle in good working order to utilize for incidental transportation required for staff and program participants to and from community activities. Incumbent may work with violent/physically aggressive participants and responds to emergency situations involving potential physical harm to self and others. Incumbent may work extended hours, evenings, and occasionally travels out-of-town as needed.
PHYSICAL REQUIREMENT- Depending on the needs of the individuals, the work can be physically demanding including transferring or supporting people with mobility needs, lifting or moving items up to 75 lbs, walking or standing for extended periods of time or providing support through CPR/First Aid, behavioral support, and other interventions. QIDP’s must be able to transport clients in a motor vehicle.
