Job Responsibilities:
- Provides individual, group, conjoint or family psychotherapy and support services to children with severe emotional disturbance and their family.
- Conducts clinical assessments, develops diagnoses, and participates in developing person/family centered treatment plans with client families.
- Monitors client progress and coordinates appropriate behavioral health and other needed services including, but not limited to, community coordination, outreach, and referrals.
- Responsible for maintaining an electronic and paper chart which includes required documentation of services provided, progress notes, treatment plans, periodic reviews and client rights documents.
- Participates in individual supervision and team meetings to review progress, identify barriers and develop strategies for better assisting families in reaching their goals.
- Responsible for full compliance of clients on caseload will have CAFAS/PECFAS completed quarterly.
- Provides 24/7 crisis intervention availability on a rotation basis to help offset potential risk of self-harm or conditions which could lead to out of home treatment.
- Ensures that paperwork is maintained in accordance with funder requirements as well as Easterseals policies and procedures.
- Responsible for assessing, reviewing all requests for services and authorizing medically necessary services for all consumers. Only implements therapeutic modalities approved by Easterseals Michigan.
- Demonstrates knowledge of principles of trauma informed care and co-occurring disorders.
- Willingness to support and assist consumers with trauma related issues.
- Confirms that all individuals served caseload have CAFAS/PECFAS completed quarterly.
