Primary Purpose of Position: This position supports, conducts and coordinates transition to community living activities for individuals currently residing in facilities identified as Institutes for Mental Disease (IMD’s) who want to move into independent community living situations and for individuals seeking other housing opportunities within their communities. Assists with planning and development of affordable and assisted housing and related support services. This position will assist in the development of a community integration plan and connect the individual with appropriate services and supports and will provide Supportive activities to ensure a successful transition. This position will act as advisor on affordable and supportive housing programs for Eastpointe’s members and assist as needed with landlord recruitment and retention
Responsibilities and Duties:
- Work closely with Transition Team and other care managers to identify appropriate housing for individuals.
- Assists in developing an effective written transition plan to enable the individual to live independently in an integrated community setting. Works with the individual and the individual’s family and supports to develop a thoughtful, organized transition plan that addresses his/her community-based support needs.
- Works with the transition/care plan team in implementation of person-centered planning processes in which the individual has a primary role and is based on the principle of self-determination.
- Assists members gain access to needed Medicaid and/or State Plan services, as well as medical, dental, social, housing, educational and other services and supports, regardless of funding source for the services or supports to which access is gained. Perform activities to support the individual’s successful integration within the community. Assists the individual, his/her family and supports to identify and secure the community resources necessary to transition.
- Assists in identification of barriers to transition and work with team to identify methods of assuring accessibility of identified recipients/members.
- Participates in the three-month post-transition follow up plan for each transitioning individual based on individual needs and circumstances.
- Works within the organization’s policy and procedures and accreditation standards.
- Meet with members to complete budgets for available funding sources.
- Completes Purchase Order Requestions, ensure all financial transactions are complete and submitted.
- Assist member with obtaining furniture, food, and household supplies and utilities.
- Assist member with housing searches, request inspections, complete required documentation with landlords.
- Research and establish new relationships with landlords
- Completes QOL’s and Biennials timely.
- Link, Monitor, and assist members with providers.
- Participate in treatment team meetings with hospitals and providers and assist with PCP/ISP/Care planning.
- Coordinate with Adult Care Homes for members transition into the community.
- Coordinate with DSS for SA In-Home other needed resources.
- Coordinate with DSOHF admission/discharge coordinators and other care manager staff for available housing options
- Act as advisor on affordable and supportive housing programs for all Eastpointe staff
- Participate in Quarterly DHHS Housing Specialist calls
- Work with In reach staff for coordination of housing
- Confirm members source of income via DSS and or Social Security Administration.
- Assist members with establishing Payee services, as needed or requested
- Ensures that CLIVe and Social Serve are up to date as the members transition progresses for TCL members only.
- Coordinate meetings with ADANC
- Coordinates initial moves and rehousing as well as assisting members with Bridge housing.
- Link members with supportive employment.
- Complete IEV and Spend down for end of month reporting.
- Other duties may be assigned as needed.
Education/Experience/Licensure:
Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area or licensure as a registered nurse (RN). Must have two (2) years of experience working directly with individuals with BH conditions.
