Position Description: Transition Support Specialists Supervisor provide support to individuals being discharged from Institutes for Mental Disease (IMD) into community based independent living situation or diverting from institution and supervise all agency employed Transition Support Specialists. The position would supervise the transition support specialists and provide backup coverage to staff that are out with any current job functions. The supervisor would help staff with assisting individuals in identifying the services in their communities that will assist them in living independently, in addition to providing oversight of day to day activities, monitoring caseloads, and auditing staff work to maintain compliance with the Department of Justice (DOJ) settlement and contact requirements. The In-reach function is an engagement, education and support effort designed to accurately and fully inform adults who have a serious mental illness (SMI) or a serious and persistent mental illness (SPMI) about community-based mental health services and supported housing options including but not limited to; the availability of tenancy support services and rental assistance. Services range from identifying locations of appropriate service providers to teaching individuals community skills needed to be successful in living as part of a community.
Education/Experience/Licensure: Master’s Degree in human service field or related degree and fully licensed LCSW, LMFT, LCMHC or RN licensure are required for this position; and three (3) years of experience providing care management, case management, or care coordination to the population served. Position requires substantial progressive managerial experience. Must reside in North Carolina.
