PURPOSE OF YOUR JOB DESCRIPTION


The primary purpose of your position is to assist residents in activities of daily living that are outlined in this job description, and may be directed by your supervisors, i.e., Preceptor, Neighborhood Charge Nurse, or Clinical Mentor.


JOB FUNCTIONS

MAJOR DUTIES AND RESPONSIBILITIES


  • Ensure Residents identity before administering service, serving meals, etc.
  • Transport residents to and from dinning services, activities and any other place they would like to go
  • Report all changes in the resident's condition to the Charge Nurse as soon as practical
  • Report all accidents/incidents you observe on the shift as they occur
  • Greet residents with a smile and respect
  • Make resident comfortable
  • Inventory and mark the residents personal possessions as instructed
  • Assist residents with packing their personal possessions when they are being transferred to a new room or when being discharged
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment throughout the neighborhood and shift
  • Report all complaints and grievances made by residents to the Charge Nurse
  • Follow established policies concerning exposure to blood/body fluids
  • Apply lotion to skin
  • Change bed linens and make beds. Keep lines tight to avoid wrinkles from forming under the resident
  • Put extra covers on bed as requested
  • Assist residents in preparing for activity and social programs (i.e., church services, parties, visitors, etc.)
  • Assist in transporting residents to/from appointments, activities and social programs, etc., as necessary
  • Answer resident call lights promptly
  • Stay in bathroom with residents that are DNLA
  • Ensure that residents who are unable to call for help are checked frequently
  • Check each resident routinely to ensure that his/her needs and wishes are being met
  • Visit with residents who cannot or will not attend activities.
  • Take menu orders from residents
  • Prepare residents for meals; wash hands, position tables, place clothing protectors, take to/from dining room, etc.
  • Serve food trays, Assist with cutting foods, providing condiments and assist in dining room supervision
  • Assist residents with identifying food arrangements (i.e., informing resident with vision problems of foods that are on his/her tray, where it is located, if it is hot/cold, etc.)
  • Assist with feeding
  • Keep residents water containers filled with fresh water at least every eight hours and within easy reach of the resident
  • Serve between meal and bedtime snacks and prepare snacks as assigned
  • Perform after meal care; remove trays, clean residents hands, face, clothing
  • Check rooms for food articles (i.e., food in proper containers)
  • Keep the nurses' call system within easy reach of the resident
  • Immediately notify the Charge Nurse of any resident leaving/missing from the facility
  • Handle and prepare foods and fluids in accordance with sanitary regulations
  • Assisting in documenting food acceptance
  • Stock supplies
  • Perform all other duties as may be assigned by your supervisor

Sanitary Requirements


  • Wash hands before and after performing any service for the resident
  • Follow established safety precautions in the performance of all duties
  • Keep residents personal possessions off the floor and properly stored.
  • Keep floors dry. Wipe up spills immediately
  • Keep excess supplies and equipment off the floor. Store in designated areas.
  • Wash wheelchairs, walkers, etc., as instructed
  • Clean, disinfect, and return all resident care equipment to its designated storage area after each use
  • Perform routine housekeeping duties (i.e., clean bedrails, over bed table, night stand, etc. that relate to nursing care procedures.)
  • Report all hazardous conditions and equipment to the Neighborhood Charge Nurse immediately
  • Report all safety violations
  • Follow established procedures in the use and disposal of personal protective equipment

Equipment and Supply Function:


  • Use only the equipment you have been trained to use
  • Operate all equipment in a safe manner
  • Use only the equipment and supplies necessary to do the job. Do not be wasteful
  • Report defective equipment to the Neighborhood Charge Nurse
  • Inform the Neighborhood Charge Nurse of your equipment and supply needs



Resident Rights:


  • Maintain the confidentiality of all resident care information
  • Ensure that you treat all residents fairly and with kindness, dignity and respect
  • Ensure that all nursing care is provided in privacy
  • Knock before entering the resident's room
  • Report all grievances and complaints made by the resident to the Charge Nurse
  • Report all allegations of resident abuse and/or misappropriation of resident Property to Charge Nurse

Working Conditions:


  • Works throughout the nursing service area (i.e., nurses stations, resident's rooms, etc.)
  • Sits, stands, bends, lifts and moves intermittently during working hours
  • Is subject to frequent interruptions
  • Is involved with residents, personnel, visitors, government agencies/personnel, etc.; under all circumstances
  • Is subject to hostile and emotionally upset residents, family members, personnel, and visitor
  • Communicates with nursing personnel and other department personnel
  • Is subject to falls, burns from equipment, odors, etc., throughout the workday
  • Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses

Specific Requirements:


  • Must be able to speak, read and write the English language
  • Must possess the ability to make independent decisions when circumstances warrant such action
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
  • Must possess the ability and willingness to work harmoniously with professional and non-professional personnel
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to incorporate them into existing nursing practices
  • Must be able to relate information concerning a resident's condition
  • Must not pose a direct threat to the health and safety of other individuals in the work place
  • Must be able to move intermittently throughout the work day
  • Must be able to cope with the mental and emotional stress of the position
  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that requirements of this position can be fully met
  • Must function independently and have flexibility, personal integrity and the ability to work effectively with residents, personnel and support agencies
  • Must be in good general health and demonstrate emotional stability
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times, hostile people within the facility
  • Must be able to assist in the evacuation of residents during emergency situations
