Performs duties related to, but not limited to access, initial assessment and case management of families, adults and/or children seeking or receiving services from the Department of Human Services
EXAMPLE OF DUTIES
Works with adults and families to assess and develop goal oriented, time limited, strength-based service plans to address identified needsWorks to protect vulnerable adults and children, preserve families, or to keep out-of-home placement at minimum effective levelsWorks to achieve active involvement and participation of individuals, family or significant others involved in the service programIdentifies, through case management and/or access services, individual needs which include medical, psychological, vocational, social, familial, and daily living spheresExplains the consequences of non-compliance with court orders to adults and families who are involuntarily referred, monitors utilization of services, and informs the court of adherence to court ordersFacilitate adult and families' access to services, monitors cases to ensure that services are provided in an appropriate and timely manner, reviews progress, and terminates services when stated goals are reachedTeaches and advises individuals and families about available community resources and how to independently gain access to needed servicesWorks to achieve services and treatment objectives without duplication of effort and with maximum effective use of staff time and available resourcesMaintains a complete record for all assigned cases; completes required written assessments, reports, and other related paperwork as necessaryDocuments contacts and services, maintains files and data for reporting purposes, prepares court reports, collateral dictation, and prepares other records and reportsAttends and participates in staff meetings, supervision times, and other regularly scheduled meetings; develops professional knowledge and skills by attending training events, conferences, and workshopsServes as advocate for clients within the communitySpeaks to the community and other organizations to provide public educationPerforms other duties as assigned

Paid or un-paid experience in a County Department of Human Services handling child protective services, adult services case management or service facilitation.
