Position Description:
Under the direction of the Director of Programs, the Case Manager will provide social services and other assistance to improve the stability of low-income families and to maximize the well-being which leads to self-sufficiency. This position will assist clients in removing barriers to access childcare for their dependent children. This position will assist with referrals for counseling, community resources, government programs, and other resources for families and children.

Duties/Responsibilities:

  • Interview clients individually, families or in groups to assess their childcare needs, capabilities, and problems to determine what services are required to meet their needs.
  • Collect data, tracks client progress, and completes reports as required.
  • Identify the need for intervention, provide guidance and refer participants to available resources.
  • Assess client’s needs to develop action plans for achieving self-sufficiency.
  • Develop and review goal plans in consultation with clients and perform follow-ups assessing the quantity and quality of services provided.
  • Facilitates program overviews.
  • Serve as an advocate for parents and childcare providers.
  • Participate in staff meetings, cases reviews, and file checks.
  • Follow-up 30, 60, 90 days or greater when needed.
  • Interview clients to gather information about their backgrounds, needs, or progress.
  • Give full attention to what other people are saying and take time to understand their perspectives.
  • Document client information into ELOGIC and HMIS systems in a timely manner.
  • Respond to messaging in a timely manner via phone, email, etc.

  • Ability to use good judgement and make sound decisions.
  • Ability to speak effectively to all groups.
  • Ability to de-escalate situations with clients.
  • Ability to give counsel to case managers.

Qualification Requirements:

Education and/or Experience

  • Two years’ experience in a Case Management support position. Must have at least a minimum of 3 years’ experience working face to face with the public.
  • Language Skills: Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar. Must have proficiency in speaking, reading, writing, spelling, rules of composition, and grammar. Spanish speaking individuals are preferred. The ideal candidate for this position must have a good understanding of diversity and cultural awareness.
  • Mathematical Skills: Ability to apply basic mathematics to accurately monitor budgets.
  • Reasoning Skills: Ability to solve practical problems and deal with a variety of variables in situations where limited standardization exists. Ability to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.

Certificates, Licenses, and Registrations

  • Must be trained on the principles and procedures of Results Oriented Management and Accountability (ROMA).

Job Type: Full-time

Pay: $19.30 - $21.70 per hour
