Director of Clinical Services | Geriatrics

They employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care, which is key to organizational values, team culture, and mission.

We seek a Bachelors or Masters educated California RN with three (3) or more years of supervisory experience with demonstrated aptitude to mentor and develop team members for the role of Clinical Services Director. The person in this role will be accountable for the delivery of high-quality clinic, social work, behavioral health services, and rehab services. She/he will provide ongoing clinical guidance and support to the IDT, as well as provide back-up planning and facilitation of the IDT meetings as needed. She/he will direct clinic, social work, and rehab operations including hiring, training, supervising, and mentoring team members and ensuring clear and effective workflows. She/he will ensure patients experience smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.), deploy team members as needed, and work with the Complex Care Team, Center Director, and Medical Director. She/he will achieve key performance indicators as outlined in monthly operating reviewing and weekly tactical meetings.

Candidates must have an Bachelor’s or Master's degree in nursing and be licensed as an RN in the state of California. Must have experience within the last five (5) years in an outpatient care clinic or health facility. Must have strong knowledge and experience to treat participants and oversee complex clinical needs, particular with the frail and elderly population. 

The role includes highly competitive compensation (salary to $185,000 based on experience) and Medical insurance coverage (Medical, Dental, Vision) as well as 17 days of personal time off (PTO), 12 holidays observed annually, and sick time. 401 K savings match also offered.
