JOB DESCRIPTION
This position requires that the Licensed Clinical Social Worker provide linkage and resources to all of the clients needing services in Eisner Health’s Clinics. It requires close collaboration with the clinicians treating the patients as well as the ability to build relationships with these clients. It also involves counseling and providing integrated services as part of this primary care team.
This position requires the full understanding and active participation in fulfilling the mission of Eisner Health. It is expected that the employee demonstrate behavior consistent with the core values of Eisner. The employee shall support Eisner’s strategic plan and goals and direction of the performance improvement plan. The employee will also be expected to support all organizational expectations including, but not limited to; Customer Service, Patient’s Rights, Confidentiality of Information, Environment of Care and Eisner Health initiatives.
ESSENTIAL DUTIES
1. Provides brief biopsychosocial assessments, care plans and Therapeutic interventions to patients in need of behavioral health support.
2. Experience in working with adolescents and adults in a therapeutic setting.
3. This role supports 2 sites: Santee High School and Lynwood Clinic
4. Completes reporting process in cases of rape, domestic violence, child abuse, and elder abuse.
5. Assists and arranges referral services and/or shelters for victims of domestic violence or elder abuse.
6. Intervenes in cases involving difficult patients/families, or clients demonstrating behavioral problems.
7. Responds to requests from other departments within the Center to meet with a potential client or to provide case management services as needed.
8. Provides referral services to patients in need of housing, food, clothing or ancillary medical services, such as optometry referrals, etc. either in the Center or through referrals to community agencies.
9. Arranges transportation services for patients.
10. Assists patients in completing applications for any financial assistance programs, USCIS (Immigration and Naturalization), or legal services.
11. Connects patients with internal and external health education programs or classes. May conduct group classes or programs for patients as appropriate.
12. Provides initial paperwork and psychosocial assessment for client initial intake as appropriate.
13. Collaborates with the medical and nursing providers to formulate, implement and revise discharge planning. Works closely with clinical staff in coordinating and obtaining needed services for patients and families.
14. Provides psycho-educational support for patients. Working with the medical or social worker provider, designs specific behavioral plans for the patient.
15. Obtains medical records from other facilities as requested.
16. Provides case management and follow up of special cases involving clinic patients, including those who are admitted to the hospital, HMO patients who are out-of-area; or those who are referred out for other services.
17. Acts as interpreter for non-Spanish speaking clinicians as needed.
18. Documents all client contacts according to clinic standards.
19. Participates in all mandatory meetings.
20. Attends classes or meetings for professional development.
21. Maintains productivity expectations.
22. Assumes other duties as directed by department Clinical Director
23. Adult Department Specific Duties: Acts as the principal person to arrange a PET team or psychiatric 911 emergency contacts in the event of a suicidal crisis. As a sitter for suicidal patients. Evaluates and counsels prenatal patients per CPSP protocols.
POSITION REQUIREMENTS
1. Must be licensed as a LCSW with the CA Board of Behavioral Sciences
2. MSW (degree in social work) preferred. Master’s Degree in psychology may be considered.
3. Possess a Current CPR Certification.
4. Fluency in English and Spanish required.
5. Experience working with diverse populations in a social service setting highly desirable.
6. Previous experience interpreting desirable.
7. Experience in working with Electronic Medical Records, preferred.
