What are the responsibilities and job description for the Infusion RN position at Elevation Infusion Services?
Elevation Infusion Services is looking for a Per Diem Infusion Nurse who provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site setting.
Essential Duties and Responsibilities for the Registered Nurse (RN):
- Provide the majority, if not all of the direct care for all specifically assigned patients including initiation and maintenance of all communications with appropriate care team members including pharmacists, physicians, dieticians, home patient representatives and any appropriate community resource staff members.
- Evaluate and educate the patient/care partner to safely administer the home infusion therapy and provide additional instruction as indicated.
- Evaluate the home environment where the care is to be provided and identify interventions that may be required to render the home care environment safe and appropriate for the prescribed care.
- Develop, in conjunction with other clinical team members, each patient’s Care Map and evaluate the patient's response to therapy during each home visit or encounter.
- Appropriately discharge primary patients and complete discharge summary within 7 days of discharge.
- Develop and maintain caseload scheduling and communications on a weekly and monthly basis in updating schedules with changes. Schedule home visits with a patient/care partner and document the patient visit schedule calendar, which is posted in the patient's home.
- Document patient visit to physician or lab drawing station on calendar.
- Coordinate care via live telephone conversations and/or voicemail with the team as determined by the urgency of information to be relayed.
- Document all communications at the time of service.
- Takes part in the nursing on-call rotation in order to provide continuous nursing support services to our customers.
Requirements for the Registered Nurse (RN):
- RN Degree, BSN Preferred
- Minimum 3 years nursing experience with background in Infusion therapy a must
- Current nursing license for applicable state
- Experience in ICU and/or Cardiac Care preferred
- Excellent customer service skills
- Proficient with Microsoft Excel, Word and Outlook
- Proficiency insertion of peripheral IVs
- Must have a valid driver's license and auto insurance
- Carry own malpractice insurance
- BLS certification.
IV Infusion Competencies Requirements:
- Critical thinking skills
- Phlebotomy
- IV placement, including preferral, central Picc, and midline
- Dressing changes
- At least one year of experience
Job Types: Part-time, Per diem
Pay: $65.00 - $75.00 per hour
