What are the responsibilities and job description for the Registered Nurse ACCS position at Eliot Community Human Services?

Responsibilities:
- Provide education and consultation to Persons and staff regarding medical health, medication and medical topics. Provide 1:1 training in specific individualized need areas.
- Develop and conduct groups and trainings for Persons and staff in general areas of health and wellness including nutrition, exercise, smoking cessation. Assist Persons with accessing community resources that will support and assist with addressing needs and health wellness efforts.
- Provide oversight, guidance and assistance to Persons with understanding their medical conditions. Meet with Persons and staff to discuss any medical diagnoses and conditions, causes, symptoms, complications, and treatment options.
- Perform direct medical care for a Person as appropriate and ordered by physician which may include monitoring of vitals, wound care, etc. This includes providing interventions specific to Person’s TP (treatment plan) for medical/health and wellness goal areas.
- Review all medical documentation for each Person as assigned and provide medical care consultation and coordination as needed. Provide assistance coordinating overall medical care for Persons and ensuring medical follow up is done as needed. Ensure compliance with MAP.
- Provide medical consultation and direction as active member of assigned committees, including the Agency’s risk committee and Health and wellness committee.
- Provide side by side interventions to Persons that focus on education and skills training in relation to health and wellness issues. Assist the team and Persons by providing methods for on-going assessment of symptoms and treatment effectiveness and identify appropriate responses/actions.
Schedule: 9 AM -2 PM Mon, Tues, Thur, Fri, Sat
Experience
Preferred- 2 year(s): Experience working with adults with serious or persistent mental health issues.
Education
Preferred- Bachelors or better in Nursing or related field
