Daily On-Site Job Responsibilities of LVN/MA
· Signal Portal
Communication with Medical Provider for Zooms, Medication adjustments, Change of Condition, and relay documentation issues
· Pharmacy Portal
Order medications, communication with pharmacy staff for medication issues
· Take Vital signs per protocol and as needed - input in KIPU
· Assess Cows/Ciwas per protocol and as needed - input in KIPU
· Overseeing self-administration of scheduled and prn medications
· Medication – orders, refills, adjustments, changes, centrally store, input, witness/verify medications brought in.
· Controlled Medications – log in Controlled book, centrally store, input as taper, upload of receipts and completed controlled logs into medical tab
· Upload Client medical documents/discharges into medical tab
· Input Client infection control for dx/medications in monitoring log
· Nursing progress notes – admission, residential discharge, final discharge, change of condition, medication change, additions, discontinuations, and client teaching
· Final and Residential Discharge – communicate with the provider for discontinue or continue of medications in SL or home environment, add current medications and Nursing Progress notes
· Outpatient Medication tab – Open tab and populate with current medications upon residential discharge
· Daily Nursing End of Shift Report (EOSR) via email to all staff – for continuity of care
· Daily communication with Celeste for updates/issues/endorsements
Weekly On-Site Job Responsibilities
· Coordination of Clients for Weekly Assessments – with PA
· Carry out medication changes provided in assessments
· Endorse to Celeste - labs, referrals, prior authorization medications
· Monitor controlled logbook for errors on last day of shift – current week
· Off site on call availability (4hours) for medical issues/questions when Director of Nursing is not available 24/7
Monthly On-Site Job Responsibilities
· Attend monthly staff meetings as required by Operations Director
· Attend Medical training as needed/required
· Complete monthly staff education as required by Operations/Medical/DHCS/TJC for compliance
Requirements
1. Valid COVID Vaccine Card & Booster if eligible
2. Current LVN
3. CPR & First Aid Certification
Job Type: Full-time
Pay: $26.00 - $29.00 per hour
