The Psychiatric Social Worker II will join the Ellis Outpatient Mental Health Clinic. The mission of the clinic is to provide person-centered, holistic, recovery-oriented care so individuals can achieve their life goals. This employee will have the opportunity to provide direct care and advocacy for a diverse and vulnerable population. The clinic team specializes in a wide range of treatment interventions, including trauma-informed care and integrated mental health and substance use treatment.

 

EDUCATION AND EXPERIENCE REQUIREMENT(S):

  • Master’s Degree in Social Work
  • NYS LMSW licensure required
  • Mental health experience strongly preferred

PHYSICAL REQUIREMENTS:

 The position is located indoors under normal working conditions.  Should be able to push/pull 25 lb., lift/move 15 lb. from floor to table, be able to perform moderately difficult manual manipulations such as using a keyboard, writing, and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers.  Mobility requirements may include the ability to sit at a computer terminal or workstation for a prolonged period of time in addition to being able to squat, stand and walk for a reasonable length of time and distance. 

PRIMARY RESPONSIBILITIES OF THE POSITION:

  • Provides direct care to a diverse population of people diagnosed with Serious and Persistent Mental Illness (SPMI).
  • Carries a caseload of 60-90 patients; the number will be calculated based on acuity and need of patients, as well clinical/supervisory judgment. Most patients served at the clinic are higher need/higher acuity individuals.
  • Primary responsibility is to provide individual psychotherapy based on the treatment need of each individual patient; this includes providing evidence-based treatments such as CBT, DBT, ACT, psychodynamic psychotherapy, and other clinically appropriate interventions. Option to facilitate clinical groups, including trauma and substance use-related groups under the supervision of track leaders.
  • Advocates for an underserved population through coordination with primary care, care management, and other community partnerships.
  • Assess determinants of health and make referrals for appropriate services, including but not limited to housing, transportation, medical providers, and care management.
  • Attends individual supervision (frequency determined by supervisor), bimonthly group supervision, and weekly team meetings with the MD/NPP staff.
  • Facilitates and/or attends patient-centered meetings with community providers required for optimal patient care.
  •  Performs initial assessments on new patients over the course of 1-3 sessions, makes formal diagnoses and recommendations  for treatment .
  • Utilizes the CASE (Chronological Assessment of Suicide Events) approach to assess suicide/homicide risk and safety  to provide accurate interventions to mitigate harm to patient and community.
  • Submits all documentation within 72 business hours and ensure yearly treatment plans are up-to-date with each billable service.
  • Adheres to all OMH, Joint Commission, Department of Health, Medicaid & Medicare regulations.
  • Coordinates treatment with  treatment team to ensure high quality patient care.
  • Provides clinical services to patients who are on Assisted Outpatient Treatment (AOT) or other court-mandated treatment through the County and/or judicial system; this includes writing the AOT orders for the treatment team, submitting them to the county, and coordinating/attending all AOT meetings.  
  • Serves as the back-up clinician at the clinic 1-4 days per month, including providing crisis intervention, de-escalation, and risk assessment to any clinic patient or community member who requires immediate assistance.
  • Perform other duties as assigned.

Salary Range:  $29.44-$44.16/hour     Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
