To be a successful KEEP Clinic Liaison you must become able to do the following always:
Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
Be the liaison between KU Med Center and the company for the KU & EmberHope Engagement with Psychiatry (KEEP) Clinic Work with the clinic staff ensuring appointments is made and attended.
Support the permanency, treatment, and cross-over case management teams by scheduling appointments and interfacing with the KEEP Clinic.
Provide transportation to youth to the KEEP clinic if needed.
Request and participate in all complex case staffing’s along with members of the youth’s permanency team.
Schedule the complex staffing with a multidisciplinary team i.e., KEEP Clinic staff, the youth’s permanency team members, clinical services director, DCF PPS Administration, placement provider, KDADS Behavioral Health and Home and Community Based Services Commissions, CMHC/CCBHC, CDDO, PRTF and the MCOs.
Participate and coordinate other stakeholder meetings and the KEEP Clinic staff as requested.
Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
To qualify for this job, you need to have:
Bachelor’s degree in social work or a related degree.
License – BSRB license preferred.
Work Experience – Two years of experience working in child welfare, mental health clinic or related fields.
Strong organization and time management skills.
Demonstrate knowledge and skills necessary to provide age and level of care-appropriate assessment and care relative to the client being served.
21 years of age. Must pass a background check, fingerprinting, and physical and drug screening.
Maintain company-required training.
Ability to travel throughout the state.
