What are the responsibilities and job description for the Coordinator position at Empath Health?
The ADC Coordinator :
- Coordinates the daily tasks at the day center and assigns them to the program aides to complete.
- Training and support of new program aides that onboard.
- Is an active participant in the Interdisciplinary team and discusses any changes with the interdisciplinary team of those participants that attend the center.
- Will problem solve immediate issues that need to be addressed with a solid resolution.
- Will be active role in the care plan development of those that attend to the center including their social and personal care needs.
- Ensure the coordination of breaks and lunches to ensure consistent coverage in the day center.
- Assists as needed with the implementation of activities and any personal care tasks.
Qualifications, Skills and Abilities :
High school diploma or GED required. CNA / HHA certification preferred.Experience working on an Interdisciplinary Team in a PACE or Community-Based healthcare setting is preferred.
1-3 years of experience with task supervision preferred.Must have a valid driver’s license, proof of insurance and have means of transportation.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Successfully complete a Competency Assessment upon hire and receive annual competency training.
