Overview

We are looking for a Director of Value Based Palliative Program Implementation to join our team. This person will be responsible for planning and managing the implementation of customer-facing programs by balancing scope, timeline, and quality to deliver customer requirements and satisfaction. This position will primarily be called upon to implement Gentiva’s new value-based palliative care offering with new customers, working with various stakeholders across a complex client base including risk-based primary care groups and health plans.

• Provides strategic thought leadership for managing deliverables and timelines in collaboration with internal and external customers.
• Work in direct customer-facing role. Work extensively with the customer’s executive, clinical, technology, and finance leadership to ensure timely implementation of Gentiva’s products and services.
• Collaborate closely with the customers’ leadership and program management team to negotiate realistic plans for successful go-lives, considering all applicable training objectives, the customers’ goals and success criteria, and any other customer-specific factors including risk and risk mitigation.
• Monitor programs daily to track progress against the schedule, resolve risks/issues and manage program changes.
• Oversee all phases of new client implementation. Lead cross-functional workstreams, establish project plans, track issues, manage deadlines, and drive on-time execution for strategic and tactical business initiatives.

About You

• Bachelor’s degree in business or related field required; master’s degree preferred.
• Value-based care experience required.
• 8 years practical and progressive experience in customer-facing healthcare program implementations.
• Strong leadership skills, with emphasis in leading projects with payers or provider stakeholders preferred.
• Project Management (PMP or equivalent) certification preferred.
• Position requires up to 25% travel.
