Position Objective: Ensure individuals with developmental and intellectual disabilities receive support and services to live their best lives. Responsible for counseling, case management, Personal Plan implementation, and structured behavior programming for people with disabilities receiving residential services. Responsible for the protection, maintenance, and enhancement of the value of commercial real estate assets to maximize the owner's financial return and benefit the tenants, consumers, and the community. This is accomplished by directing and coordinating the center's operational activities, leasing, and marketing. This incumbent represents the property owner to both the tenants and the public and will follow all rules and regulations as required by licensing entities.

Essential Duties/Responsibilities

Mission, Vision, Values

  • Support the philosophy through the agency’s mission, vision, values, and strategic plan.
  • Manage all responsibilities based on the agency mission, using individual choice and all appropriate policies, procedures, and regulations.
  • To conduct oneself to reflect the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts.

Assistance to (if appropriate)

Staff Supervision (if appropriate)

  • Directly supervise the apartment DSP staff.
  • In conjunction with the HR department, interview, hire, supervise, assign, evaluate the performance of, and when necessary, discharge assigned personnel directly and in cooperation with appropriate department heads.
  • Carry out supervisory responsibilities in accordance with the organization’s policies and procedures and all applicable laws.
  • Authorize and plan personnel requests for vacation time, leaves of absence, travel authorizations, change in position, and in-service training directly and in cooperation with appropriate department heads.
  • Responsible for planning, assigning, documenting, and directing work, appraising performance, rewarding, disciplining employees, addressing complaints, and resolving problems.
  • Assure all supervisory staff receive orientation, in-service, and continuing education to increase the performance level and maintain optimum performance standards within the department.
  • Monitor that all department staff meet performance expectations and address concerns in a timely manner to assure peak performance for individuals served and other co-workers.
  • Hold staff accountable for assigned responsibilities and meet monthly with each staff to review performance and develop a plan of correction as necessary.
  • Perform disciplinary action meetings as required.
  • Assure all intermittent sites maintain appropriate paperwork for licensure and DHS regulations.
  • Assure all personnel are registered, licensed, and certified.
  • Ensure that adequate and appropriate personnel coverage is maintained on-going.
  • Participate in the on-call rotation.

Program /Department Development, and Coordination

  • Provide Quality Assurance for the condition of homes through inspections as needed.
  • Work with all members of a person’s team, including but not limited to other EP!C team members, guardians, and ISC.
  • Assure all safety and emergency procedures are conducted calmly, accurately, professionally, and documented.
  • Participate in discovery meetings for all people on their caseload.
  • Act as a positive role model for others.
  • Assure dignity, privacy, and rights of people are always observed.
  • Assure that activities are developmentally and age-appropriate and follow principles of normalization.
  • Assure that people’s needs and interests are assessed continuously.
  • Assure that the above findings are utilized for planning for each person.
  • Assure that the personal and/or social development of people supported occurs.
  • Maintain a QIDP caseload as needed when changes occur with QIDP caseloads as designated by the Director.
  • Coordinate the admission, transfer, and discharge as needed for people.
  • As a mandated reporter, ensure allegations of abuse, neglect, and exploitation are reported as required by EP!C policy and government regulation.
  • Conduct prompt and thorough investigations for all allegations, documenting as required.
  • Act as the company's primary coordinator to ensure that the company’s efforts fully meet and exceed contractual property management obligations.
  • Develop a network of emergency services and be available for weekend coverage as required.
  • Supervise on-site maintenance mechanics, if utilized.
  • Manage tenant relations and coordinate requests for repairs and maintenance, understanding of rent charges, lease issues, and other tenant concerns.
  • Prepare for team meetings, in advance, and act as chairperson for the meeting.
  • Keep abreast of new technologies, systems, and procedures related to property management.
  • Provide needed transportation to people supported using an EP!C vehicle or their vehicle as required.
  • Meets all deadlines set by the agency and outside resources.
  • Understand rules governing program/licensure for Intermittent CILA.
  • Maintain positive parent and guardian relations for individuals on the team.
  • Promote community participation for individuals.
  • Review all medical and other incident reports for the person.

Fiscal Management

  • Use resources responsibly.
  • Work with the supervisor to seek approval of expenses as required.
  • Enforce tenant compliance and perform landlord obligations.
  • Occasionally respond to routine leasing inquiries.
  • At all times, keep the space in condition to show.
  • Assist in the collection of delinquent accounts.
  • Communicate with the accounting department to ensure the accuracy of accounts receivable.
  • Engage, contract, supervise, and approve invoices for all goods/services required to maintain the property up to company standards.

Database, Paperwork and Compliance Responsibilities:

  • Complete monthly summaries and complete all documentation to meet clinical and financial requirements.
  • Maintain files according to standards set by the agency, ICAPs, consents, assessments, and other needed paper or electronic documents.
  • Review all documentation for accuracy.
  • Understand rules governing program/licensure for CILA. Monitors the implementation and adherence of these rules.
  • Complete and required accounting procedures in a timely manner.
  • Supports database management systems.
  • Assist with OIG investigations.

Resource Management Responsibilities:

  • Provide support to and work with all team and agency members.
  • Maintain positive guardian, family, and natural support relations for people supported.
  • Coordinate and support appointments with the nursing department.
  • Work with ISC’s
  • Assist new individuals in the transition process.
  • Communicate and serve as an agency liaison to parents, people we support, peers, community, neighbors, and work groups.
  • Develop and maintain natural support systems and promote individual community participation.
  • Audit compliance with meeting required operational standards during home visits.

Clinical and Behavioral Management Responsibilities:

  • Effectively facilitate and lead EP!C team meetings so outcomes are clearly identified and recorded.
  • Implement Person-Centered Plans (PCP) for all people on caseload, including writing and entering implementation strategies, and assist in facilitating yearly Discovery meetings, along with other meetings as appropriate.
  • Communicate regularly with people supported, family/guardian, nursing, and other team members regarding progress toward outcomes, health and safety, and other issues that may arise.
  • Achieve desired outcomes based on individual choice as delineated in the person’s PP
  • Assist with compatibility assessments with people on an ongoing basis.
  • Provide training (OJT’s) and coaching to agency staff in the principles and practices of Implementation Strategies.
  • Train staff on Behavior Plans/Interventions related to the person.
  • Develop behavior plans for people who are in need of behavioral support from their support team.
  • Work with all members of a person’s team, including but not limited to other EP!C team members, guardians, ISC, BCBA, and psychiatrist.
  • Participate in the Professional Review Committee and Human Rights Committee as needed.
  • Participate in all needed appointments with a person’s medical team and psychiatrist.
  • Participate in the on-call rotation.
  • Assist with investigations as requested.
  • Participate in Community Options vacations, power team activities, and other events that support the Community Options program as requested by the supervisor.

Clinical Management Training:

  • In accordance with agency policy and procedures, support the agency’s DSP training in role modeling, coaching, mentoring, and participating in on-the-job training with program staff.
  • Train all aspects of clinical responsibilities, including incident reports, Personal Plan, monthly summaries, drill sign-off, and other clinical tasks.
  • Provide training and retraining as needed for agency personnel in the efficient use of electronic data entry and communication (Therap, Quantum & ECP)
  • Provide training and coaching to agency staff in the principles and practices of person-centered planning.

Personal and Professional Development:

  • As requested, Participate in meetings, committees, conferences, seminars, and in-service training sessions.

Personnel Relations:

  • Interact with all departments and EP!C staff, with individuals from monitoring agencies/committees, parents, family members, and other professionals in a professional manner.
  • Facilitate intra- and interdepartmental communication on an ongoing basis.
  • Provide backup to other CO teams in the QIDP role.

Marginal Responsibilities

  • Perform other duties as assigned.
  • As a mandated reporter, ensure allegations of abuse, neglect, and exploitation are reported as required by EP!C policy and government regulation

Education/Experience Requirements

  • Bachelor’s degree in human services required, and two years of supervisory experience preferred.
  • 4 years’ professional-related experience in property management.
  • An equivalent combination of education and experience is considered.
  • One year experience with persons with developmental disabilities.
  • The ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write correspondence.
  • Successfully complete CPR, First Aid, Infection Control, and Crisis Prevention Institute (CPI) training and maintain these certifications.
  • Must obtain and maintain a valid driver’s license and automobile insurance.
  • Must complete DSP training in accordance with the state regulations and maintain acceptable status on the Health Care Worker’s Registry under the Healthcare Worker’s Background Check Act.
  • Must obtain/maintain standing as an Authorized Direct Support Person for medication administration.
  • Achieve and maintain QIDP status per DHS regulations.
  • Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.
  • Must possess emotional stability to handle stressful situations on a regular basis and the ability to cope with illness, death, and dying.
  • Must possess good organizational and time management skills.

  • A valid Illinois Driver’s License and insurance is required and must be maintained at all times while employed by EP!C.

Physical Requirements

  • Frequent walking and sitting.
  • Regular reaching, grasping, and standing.
  • Must be able to assist in the evacuation of residents in emergency situations.
  • Other physical activities related to an office environment.
  • Operates general office equipment, including multi-line phone, fax machine, printer, copier, and personal computer.
  • Ability to lift 50 lbs
  • Must be able to work extended and flexible hours if necessary.

Work Environment

  • Work may occur in the following settings: Home, Office, Classroom, Site, Group Homes, or Community.
  • Participate in a 24-hour on-call system.
  • Work hours/days are full-time and exempt status. Flexibility is a requirement to accommodate various Program, contractual, personnel, and organizational needs.
  • Will be required to drive a motor vehicle (EP!C’s and employee’s) as part of his/her employment responsibilities with EP!C. A valid Illinois Driver’s License and insurance is required and must be always maintained while employed by EP!C.
