The ideal candidate for this role is compassionate, flexible, self-motivated, detail oriented, and a constant learner. Under the supervision of the Chronic Care Management Supervisor, you will oversee the care of patients as they transition from one health care facility back to their home. This will be achieved by contacting patients after hospital/facility discharge to schedule transition of care office visit appointments with their PCP and ensuring that patients feel supported during this vulnerable time. This is an onsite telephonic position located at our headquarters in Southfield MI.

When you become our next Care Management – Team Lead you’ll take on:
- Managing a team of 8-10 care managers to deliver an exceptional patient experience
- Providing daily oversight to the case management team
- Providing guidance related to service delivery and documentation.
- Appropriately managing a small 70 pt case load and providing care management services
- Monitoring your team’s monthly performance goals
- Tracking key performance indicators
- Completing all new hire and remedial training
- Working collaboratively with members of the interdisciplinary team
- Attending staff meetings and training workshops as required
- Supporting management with protocols and processes related to company standards.
- Assessing, triaging, and directing patient issues to the most appropriate point of care
- Navigating the health system on behalf of patients to ensure they can access the care they need in the way they need it.
- Screening patients for barriers to wellness and identifying social determinants of health needs.
- Coordinating appropriate follow up appointments within EPIC Health System to ensure our patients receive the very best continuous care
- Supporting patients and their families, and providing compassionate care

You would be a great fit for this role if you have:
- Current and unencumbered LPN license
- One year of care management experience
- Three years of clinical work experience
- One year in a leadership role managing the performance of a team.
- Strong organizational and time management skills
- Proficiency at digital literacy including email and EHR.
- Proficient in Microsoft office with an emphasis on Excel
- Excellent verbal and written communication skills
- Strong problem-solving skills.
- A strong desire to innovate and streamline processes.
- A big desire to truly make a difference in your community.
- The ability to thrive in a fast-paced environment.
- The willingness to try new things and pivot when you see an opportunity to improve.

Salary : $63,500 - $80,400
