CDPAP Out Reach Specialist

Eprine Home Care seeks energetic Bilingual individual with a magnetic personality to work in our fast growing CDPAP department. Must have HHA Exchange Experience

Responsibilities:

• Match, assign, and schedule caregivers based on skill level, availability, and customer requirements
• Provide superior customer service and compassion for new clients and their families
• Receive and process incoming requests for new home care services
• Ensure compliance with all regulatory, as well as, internal policies and procedures
• Work with Medical Offices to secure medical information on patients looking to enroll in program
• Work collaboratively with various departments within the company to facilitate hiring quality caregivers
• Working with Caregivers on HHA Exchange

Requirements:

• Superior customer service and strong communication skills (written and verbal) outgoing respectful personality
• Must be able to multi-task and work with HHA Exchange
• Prior administrative / office experience is strongly preferred

• This position calls for assisting tasks of a growing department
• Educate and guide potential Personal Care Assistant's on CDPAP
• Coordinate Personal Care Assistant on boarding
• Intake on CDPAP consumers
• Manage weekly schedules on CDPAP consumers to maintain clock in /out based on EVV
• CDPAP Home visits for new intake Review all incoming referrals and conduct initial screening to ensure they meet the defined admission criteria
• Conduct informational and screening interviews with patients and their families as part of the admission process
• Enter Client- specific data into the computer system

Job Type: Full-time

Pay: $19.00 - $23.00 per hour
