SUMMARY

Assist individuals referred from any of ERLI’s referral sources to meet the basic goals assigned in their plans.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned depending on area, individuals’ personal needs, and agency needs.

Develop and implement programming designed to increase daily living skills, personal and social behaviors.

Assist in ensuring that individuals develop and understand important independent living skills such as attendance, punctuality, planning, grooming and hygiene and activities of work and daily living.

Assist individual in engaging in programming designed to increase daily living skills and personal and social adjustment.

Maintain a positive work atmosphere by behaving and communicating in a professional manner that gets along with customers, vendors, individuals, co-workers, and supervisors. This also includes guardians/parents, caregivers, providers and any other agencies you may have contact with.

Foster a pleasant, therapeutic environment.

Shape personal and social behaviors of individuals.

Provide in-home and community-based services to individuals with possible behavioral health needs, including counseling, crisis intervention, skill-building, and advocacy.

Collaborate with individual’s team to design and execute person-centered plans.

Develop a schedule of activities that meets the individuals’ program of service, areas of interest, Individual Service Plan (ISP) and person-centered plan.

Develop, implement, and document community engagement and vocational activities (i.e. outings in the community) weekly and/or bi-weekly dependent on the ISP and person-centered plan for individuals supervised.

Must drive an agency vehicle to take individuals on outings in the community.

Ensure and advocate for individual rights of the participants on a daily basis.

Recognize and respect individual differences/preferences.

Using electronic record keeping system to write and develop detailed, accurate, and SMART goals and case notes on activities for each individual assigned to your supervision. Electronic records must follow Medicaid and CARF requirements.

Document Unusual Incidents/Major Unusual Incidents (UI/MUI) in case management system. Notification to Supervisor, family, Support Administrator, and others identified required within compliance requirements.

Maintain caseload documentation of case notes, ISP revision dates, documentation, recommendations for transition to next opportunities etc. and keep supervision up-to-date regarding individual progress. Caseloads are built based on acuity, agency needs, and individual’s skills and interests.

Communicate regularly with individual’s Support Administrator through phone calls, email and meetings; keeping the Support Administrator apprised of the individual’s activities and progress in the program. Ensure all communications are documented in the case management system.

Communicate effectively to Supervisor and Support Administrator when span dates are approaching expiration. This should be communicated ideally 60 days, minimally 30 days prior to expiration to avoid no payments for providing services.

Provide on-the-job guidance and support to help move the participant toward attainment of targeted behaviors.

Use modeling techniques to instruct individuals and groups in proper work methods and positive work behaviors and how to improve work quantity and quality.

Write monthly reports regarding an individual’s progress.

Participate in annual and/or quarterly ISP meetings for all individuals supervised, as requested by supervisor.

Participate in department Program Evaluation activities.

Attend and participate in Department, Program, and agency meetings and trainings.

Recommend and implement program improvements.

Continue professional development (membership in organizations like ORA or NJSST, workshops, and seminars). Minimum requirement of 15 hours of professional development per year outside of in-service dates. Requirements can be met through DODD online and the agency offered training.

Maintain regular attendance according to ERLI Policy.

Maintain a safe work environment and follow agency safety policies.

Ensure that the participant is kept clean and unsoiled, and in the event of an accident, assist the individual in clean-up and changing of their clothes if necessary.

Maintain department area in accordance with all safety task force requirements.

Provide transportation to individual’s for appointments, shopping, outings, and so forth in an agency vehicle.

Must be willing to step in and provide NMT services based on the agency’s need.

Complete and maintain CPR/First Aid and Delegated Nurse training during entire duration of employment.

Must be willing to travel and work shifts at other ERLI locations and/or departments to assist in providing services to individuals as needed.

Other duties as assigned.

QUALIFICATIONS

EDUCATION and/or EXPERIENCE

GED or high school diploma is required; associate degree and 2 years of experience or an equivalent combination of the two is preferable. Previous experience with the DD population is required. Experience with Microsoft Word and Outlook preferred. Strong communication skills both verbal and written are required.

OTHER SKILLS AND ABILITIES

Demonstrated skills in verbal and written communication.

Demonstrated organizational and time-management skills.

Proven experience working with DD population.

Knowledge of local job market, employment trends, and skills required for a variety of positions.

LANGUAGE SKILLS

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

REASONING ABILITY

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

PHYSICAL DEMANDS

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand, walk, sit, and reach with hands and arms. The employee is occasionally required to use hands to finger, handle, or feel and stoop, kneel, crouch, or crawl. Dependent on assignment, employee must regularly lift and/or move up to 75 lbs or occasionally up to 250 lbs with team assistance or technical support. Program specific needs will been identified upon assignment.
