Job Description:
Seeking a Registered Nurse to serve as Patient Flow Program Manager. This position is based out of 32nd Avenue Building in Fargo but would work with our entire west market. Schedule is 8 am to 5 pm, Monday through Friday, 40 hours weekly. No weekend or holiday rotations are required. Responsible to provide leadership, program development and coordination between providers, staff and patients for a defined population. Acts as a primary liaison between the operations team, internal stakeholders and key community partners. In conjunction with internal departments and disciplines, coordinates community outreach programs, including screening and health promotion activities, and education. The position may include direct patient care in the role of care coordination for a defined population. Ongoing collaboration with providers, staff, quality, regulatory and reimbursing agencies are required for optimal care coordination across the continuum. Position requires knowledge and ability to research evidence-based practices analyze and interpret data to make program recommendations and improve population outcomes. If assigned as program manager over the Baby Friendly program, this role will ensure oversight and compliance with designated program requirements.
Education Qualifications:
- Bachelor of Nursing degree preferred.

Licensure/Certification Qualifications:
- Current Registered Nurse license in the state of North Dakota upon hire.
- Ability to obtain RN License in MN required within 3 months of hire.
- 3 years of nursing healthcare experience.

The approximate base compensation range is posted, but the actual offer will reflect our total compensation and benefits package.
