Home Health Registered Nurse (permanent hire position with full benefits package)
RN Home Health Job Responsibilities:
- Assess patient to determine skilled home health care needs
- Develop a Plan of Care to address patient’s needs
- Re-assessments and changes to Plan of Care as necessary
- Supervision of caregiver staff assigned to the patient
- Patient and family education for safety, best care outcomes, and to prevent hospital re-admissions
- Develop discharge planning needs when goals have been met
RN Home Health Job Requirements:
- Valid & current NYS RN license
- Skilled Home Health and OASIS experience preferred
- Acute Care experience preferred
- Ability to learn EMR software
Job Type: Full-time
Pay: $95,000.00 - $110,000.00 per year
