What are the responsibilities and job description for the Pharmacist position at Evans Memorial Hospital?
Assist the Director of Pharmacy with medication management, reviewing Physician orders for medication dosages and routes based on individual patient needs. Reviews previous and current medications to minimize adverse drug interactions, drug to drug, drug to food and drug to disease interventions. Assist with monitoring compliance and therapeutic response to drugs to ensure patients are getting correct dose and administration route. Updates patient medication profile as medication doses are changed or discontinued. Assist with monitoring antibiotic stewardship, collecting and logging data daily. Assures drug carts are stocked with appropriate medication and doses. Serves as a crucial source of information between other members of the medical and nursing staff.
Minimal Qualifications:
Must have graduated from an accredited college of Pharmacy with a BS or Pharm D. degree in Pharmacy. Must hold current unencumbered licensed issued by the Georgia State Board of Pharmacy. Minimum of two (2) years of pharmacy experience with hospital experience preferred.
Must be available to work flexible schedule to include weekends and holidays.
