Clinical Licensed Practical Nurse
Under the direction and supervision of the Clinical Supervisor, the Clinical Licensed Practical Nurse uses and practices basic nursing assessment skills(data collection), participates in planning, performs interventions in accordance with the care plan and contributes to the evaluation of individualized interventions related to the care plan. Administers medications, implements primary care orders, and evaluates participant’s complaints physically and/or by telephone. When appropriate provides health education to participant and caregiver in regards to care plan, nursing treatments or medications. Communicates participant’s condition to primary caregivers, primary care provider, and multidisciplinary team. Maintains the participant’s medical record with thorough documentation. Will insure compliance with competency requirements.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Must have a valid driver’s license, proof of insurance and have means of transportation. Must be able to meet EveryAge's auto insurance carrier's standards for coverage.
Education: Graduate of a school of professional nursing. Current North Carolina Licensed Practical Nurse (LPN) License, Cardio Pulmonary Resuscitation (CPR) certification required.
Experience: A minimum of one year’s experience in working with the frail or elderly population required.
