Join our Home Health Care Agency working one to one as a Registered Nurse in the home care setting. You can make a difference for your patients while providing critical in-home care.

Shifts-Available, all shifts in Nursing Homes and Assisted Living settings

Registered Nursing Visits- Hybrid in-person/remote:

Perform Skilled Nursing Visits including Medication Management and Administration, (Insulin, Med Pre-fil, etc), Wound Care, Chronic Disease Care Management, Catheter Care. Visits are located in Berkshire County. We cluster the visits together. Spend quality one to one caregiving with your patients. Flexible hours, working AM and PM visits, weekdays and weekends needed. Work Part-time or Full-time. You perform the in-home visits and work from home completing the visit documentation!

Private Duty Home Care Shifts:

Provide nursing care to one medically complex individual either adult or pediatric in there own home. Shifts are various hours full, part time and per diem available, the hours are flexible. You will be working one on one with the primary caregiver in the home to support and provide Skilled Nursing care to one patient in their own home.

Required:

• Register Nurse license in good standing.
• Knowledge of computers a plus
• Prefer Respiratory experience to include knowledge of trachs and/or vents, G tube care

Orientation specific to this client’s needs will be provided. 24-hour Clinical supervision is provided. We provide PPE for our staff as recommended.

In many cases, the hours can be flexible to accommodate the nurse’s schedules. Those seeking strictly per diem work are also encouraged to apply.

Must be able to document in a computer EMR system.

Responsibilities:

• Administer and treatments medications ordered by a doctor.
• Provide basic ADL care
• Provide Skilled Nursing Assessment and intervention
• Communicate with physicians and other team members.
