The Case Manager's primary responsibility will be for the delivery of case management for an assigned caseload of youth participants and their families.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
- Conducts youth assessments and creates and updates case records which include client residential location and living conditions, employment history/prospects, general psychosocial and criminal history.
- Provide case management for youth and families enrolled into the program.
- Obtains relevant social history, performs needs and safety assessments for shelter clients as they enter the housing program. Prepares service plan, assists in the completion of a personalized safety plan.
- Networks with other service providers and justice officials to facilitate client services and community response.
- Maintains positive relationships with local Job Services and other job referral agencies and community resources and acts of liaison with local employers.
- Assists in developing structured programs for job search and job retention.
- Provides case management and budgeting assistance to clients during home and office visits, offering referral and advocacy as needed.
- Develops individualized job search and retention plan for unemployed clients or family members.
- Counsels clients regarding the family intervention plan, community support vocational and employment programs, and job prospects.
- Accompanies clients to meetings with law enforcement agencies, lawyers' offices, medical providers, or other social service programs as needed. Accompanies clients or arranges for accompaniment to court hearings. When needed.
- Conducts on-site and telephone verifications of new employment as well as ongoing employment.
- Records all employment information including job terminations in client records.
- Investigates irregularities or discrepancies in client–family progress in family cohesiveness, community, and social skills development and employment needs.
- Provides current information on community resources to other staff.
- Assists supervisor in training staff and volunteering as needed.
- Documents case management activity and other client contacts in client files in a timely manner.
- Must be able to work flexible hours to meet the needs of working clients and attend an overnight, weekend camp for housing children.
- Input touchpoints (case notes, touches, referrals, etc.) into the HMIS (ETO)
- Meets weekly with Outreach Workers and other staff as needed.
- Assist with media marketing in the community and attend general council CoC meetings.
- Assist with pick up and distribution of donated goods for RHY and families.
- In accordance with the team philosophy of the agency, may routinely be required to carry out or assist with other tasks in addition to the duties listed above.
- Must act in accordance with the agency's mission statement and goals.

EDUCATION AND/OR EXPERIENCE:
- Minimum Bachelor's degree or diploma in Social Work, or a degree/diploma in a related field or a minimum of three years experience in the direct provision of community outreach activities, support, and/or education services for vulnerable populations, combined with experience in training and the supervision of volunteers.
- LSW preferred. Current Texas Driver's License required.
