For more than 35 years, BridgingLife, formerly known as Carroll Hospice, has delivered nationally recognized, compassionate care to patients and their families in our community.

We’ve expanded our services across the LifeBridge Health continuum of care to help serve people living with serious illness in the areas of palliative care, home hospice care, inpatient hospice care, and grief support.

BridgingLife is now searching for a full-time, inpatient Palliative Care Nurse Practitioner to join their team at Northwest Hospital in Randallstown, MD.

• CRNP with a minimum of 2 years NP experience in palliative, hospice or geriatric care preferred. If not currently palliative care certified, CRNP must receive certification within one year of start date.
• Ability to provide consultative visits to patients in the hospital requiring palliative care interventions; including goals of care discussions and symptom management assessment and interventions.
• Will collect health status data on palliative care patients relevant to his / her presenting problem in a continuous, systematic and complete manner.
• Will serve as an ambassador for palliative care. Coordinates the patient / family plan of care with the physicians, care managers, and interdisciplinary colleagues to promote smooth transitions in care and respect for the patients care choices.
• Will collaborate with hospice and palliative care colleagues to ensure smooth day-to-day operations within the teams. And, that the care provided to patients meets agency and regulatory quality standards, with effective and efficient resource utilization.
