What are the responsibilities and job description for the Case Manager position at Faith Health Services of GA?
Case manager will help identify appropriate providers and facilities throughout the continuum of services while ensuring that available resources are being used in a timely and cost-effective manner for both the member and the reimbursement source.
Case manager duties include:
- Maintain knowledge of policies and procedures of the state regulated SOURCE manual
- Encourage skill development of Activities of Daily Living through task completion
- Engage in respectful, open communication with individuals receiving services, the responsible party and other family and team members to promote consistent, quality care for members
- Develop effective working relationships with the individuals receiving services, the responsible party and their families while maintaining professional boundaries
- Assist in appropriate self-management involving client, including problem solving, decision making, communication, conflict resolution, anger management, social and recreational skills
- Implement goals and/or plans for clients that focus on improving their health
- Plan, monitor and evaluate actions that meet the member’s health and service needs
- Promote quality and cost-effective interventions and outcomes
- Partner with the community to provide and connect service opportunities that help improve the health and well-being of the EDWP population
- Accurately complete case notes, progress notes, document events and produce reports as per the company’s policy
Qualifications:
· Bachelor’s degree is required
- Valid Driver's License
- Reliable Transportation
· Basic computer skills (Outlook and Microsoft Word)
· Laptop

- Case management principles;
- Applicable laws, rules, and regulations;
- Available resources to clients;
- Screening and assessment principles and practices
