Stratford, CT (999 Oronoque Lane)
Hamden, CT (2440 Whitney Avenue)
Norwalk, CT (9 Mott Avenue)
Meriden, CT (1371 East Main Street)
Job Description:
The Registered Nurse/Case Manager is responsible for the coordination of all patient care and services, and for the direct quality of nursing care provided by the homemaker/home health aide in the home.
The Registered Nurse/Case Manager supports and fosters the philosophy, goals and objectives of the agency.
Reports to: Supervisor of Clinical Services
Qualifications and Skills:
- Graduate of an accredited Registered Nurse program
- Current license as a Registered Nurse in Connecticut
- Ability to assess and respond to needs of patients and families in varied settings.
- Ability to make meaningful observations and write comprehensive accurate reports.
