POSITION SUMMARY
This position is responsible for outreach and tracking the completion of visits, referrals, diagnostic tests, and laboratory tests within the quality department, in order to improve patient health outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Patient outreach for first and second visit requirements.
- Schedule all insurer identified new patients within the required timeframe.
- Tracks the completion of labs, diagnostic tests, and specialist referrals.
- Contacts patients to develop plan for completing recommended tests and referrals.
- Identifies patient barriers for completion of recommended evaluations and refers to appropriate resources. Meets patient in the community setting if necessary.
- Educate patients about rescheduling procedures and the importance of completing tests and referrals.
- Call and schedule Health Risk Assessments (HRA).

- While performing the duties of this Job, the employee is frequently exposed to airborne particles.
- The noise level in the work environment is usually moderate to quiet.

- While performing the duties of this Job, the employee is regularly required to use hands to finger, handle, or feel and talk or hear.
- The employee is frequently required to stand or sit. The employee is occasionally required to walk; reach with hands and arms; or stoop. The employee must occasionally lift and/or move up to 25 pounds.
- Specific vision abilities required by this job include close vision, color vision and ability to adjust focus.
QUALIFICATIONS:
- To perform this job successfully, an individual should have the ability to acquire knowledge electronic health systems.
- Must have adequate transportation to be able to commute to any of the Family Health Center sites.
- Ability to perform data entry and various tracking systems.
- Strong verbal and written communication skills.
- Ability to work in the community settings
EDUCATION/EXPERIENCE/CERTIFICATIONS/LICENSES:
- Associate or Bachelor degree in health related field preferred.
- Data entry or tracking systems experience desirable
